About this file
This is a Past Performance Questionnaire for RFP No. 36C24526R0030, which pertains to an electrical infrastructure upgrade project at the Huntington VA Medical Center under the Department of Veterans Affairs Veterans Health Administration. Offerors must submit three to five references from contracts or projects completed within the past five years that are relevant in type and scope. References must include the company or agency name, point of contact, and contact information. Completed questionnaires from references must be received by April 29, 2026, at 1:00 PM Local Time, directed to Aaron Morrison at aaron.morrison@va.gov, 1540 Spring Valley Drive, FMS Building 8, Huntington, WV 25704.
The questionnaire is divided into two parts: Part 1 is completed by the prospective contractor and requests information about the contractor's identity, contact details, reference client information, relevant contract numbers and award amounts, work description, and performance period. Part 2 is completed by the reference and requires the reference's name, title, signature, date, and contact information. References are asked to rate contractor performance across multiple categories including Quality of Services (compliance, accuracy of reports, technical excellence, workmanship, and problem resolution), Timeliness of Service (delivery schedule), and Business Relations (cooperative behavior, flexibility, conflict resolution, response to changes, and communication). Each rating uses three options: Acceptable (performance met or exceeded requirements with no or few corrected problems), Unacceptable (performance failed to meet requirements without satisfactory or timely correction), or Neutral (no knowledge of performance or no relevant history). References also provide an overall contractor performance rating using the same three-option scale.
