About this file

This is Attachment 7 to RFP FA330026R0020, a template form for subcontractor and teaming partner consent to release past and present performance information to the prime contractor.

The form addresses a procedural requirement for federal contract source selection: subcontractors and teaming partners must provide written consent before the Government can disclose their past performance information to the prime contractor, who is considered a private party. The Government's Past Performance Team requests that major subcontractors and teaming partners complete and submit the consent form as part of the Past Performance Volume of the proposal to facilitate performance confidence assessment during source selection. The sample letter template instructs subcontractors and teaming partners to confirm their participation in the response to the Department of the Air Force RFP and authorize the Government to discuss their performance information with the prime contractor. The form requires signature and title of an individual with authority to legally bind the company, along with the company name and address. Offerors must ensure all identified major subcontractors and teaming partners complete this form and include it in their proposal submission.

SUBCONTRACTOR CONSENT LETTER

SUBCONTRACTOR/TEAMING PARTNER CONSENT FORM FOR THE RELEASE OF PAST

AND PRESENT PERFORMANCE INFORMATION TO THE PRIME

CONTRACTOR

Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor's or teaming partner's consent. Because a prime contractor is a private party, the Government will need that consent before disclosing subcontractor/teaming partner past and present performance information to the prime during exchanges. In an effort to assist the

Government's Past Performance Team in assessing your past performance relevancy and confidence, we request that the following consent form be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent forms should be submitted as part of your Past

Performance Volume (or Proposal). (Note: Solicitation Section L – Instructions to Offerors, Volume 1, Past Performance should specify if past performance is to be submitted as a separate proposal volume).
