TABLE OF CONTENTS
L3-i General Instructions This attachment consists of two parts as follows:
Part A: Past Performance Summary – to be completed by the Offeror and submitted with Offeror’s Volume III Past Performance Proposal. Two (2) past performance summaries are required.
Part B: Past Performance Questionnaire - Offerors should complete Block 1 and submit the survey form to two appropriate customer points of contact for the two (2) Past-Performance references shown in Part A above. The customer POC’s must complete Part B of the form and return to the FAA Contracting Officer.
Offerors must follow the complete instructions for Past Performance references provided in SIR Section L.10.5.
Part A: Past-Performance Summary Instructions to Offerors Offerors must complete a Past-Performance Summary for two (2) past-performance references in accordance with Section L using the form provided on the following pages. The Past-Performance Summaries must be submitted within Volume IV of the proposal by the date and time identified in Section L.2.1 Procurement Schedule for when the Offeror’s written proposal is due.
The Offeror must complete Part A, which must not exceed 2 pages in length for each past performance contract referenced. Responses to Blocks 3 and 4 (including portions on separate pages) must be in Arial, point size 10 font. Any submission exceeding the 2-page limitation will only be evaluated up to the established page limitation.

Part B: Past-Performance Questionnaire
INSTRUCTIONS TO OFFERORS:
The Offeror must provide the Past-Performance Questionnaire (PPQ) to appropriate customer points of contact for the two (2) Past-Performance Summary references shown in Part A above. For Government references, the PPQ must be provided to two (2) references (one from the Contracting Officer (CO) and one from the program staff) for each Past Performance Summary. The program staff references should include individuals with firsthand knowledge of the Offerors performance such as a Contracting Officer’s Representative (COR) or a Task Order manager or Representative. The PPQs are due back within 10 business days.
For past performance under other contracts, such as commercial contracts, the PPQ must be provided to individuals with duties similar to those of the Government CO and COR.
To aid the person completing the PPQ, the Offeror must complete Block 1 of the PPQ to identify its name and the specific contract being referenced.
The Offeror is responsible for following up to ensure that references submit the PPQ. The due date for the completed PPQs is the same as the due date for all proposal volumes identified in Section L.2.1. The PPQ must be submitted directly by the person completing the survey via e-mail to the FAA CO identified in Section L.2.2 of the OES SIR.
Part B: Past-Performance Questionnaire (Continued)
INSTRUCTIONS TO THE PERSON COMPLETING THE PPQ:
In order to assess Offerors responding to the Federal Aviation Administration’s (FAA’s) Screening Information Request (SIR) for Ontario Engineering Services (OES), the FAA is seeking past performance information with respect to each Offeror. You have been selected as a Past-Performance reference by one of the Offerors. The name of the Offeror, contract title, and contract number are identified on the next page under Block 1. Please correct any information in Block 1 that was not accurately provided by the Offeror. Within the PPQ, the Offeror is referred to as “the Contractor” relative to its support of your organization.
You are requested to complete Block 2 plus provide a rating for Areas 1-7 in the PPQ. Narrative explanations are only requested when a rating of Marginal or Unsatisfactory is indicated.
Please complete the PPQ, sign in Block 4, convert to PDF format, and e-mail to the CO as identified in Section G.2. The subject line in the submission e-mail should clearly indicate: OES Past-Performance Questionnaire Submission for <Contractor’s Name>. The e-mail message must originate from your Government or Corporate e-mail address, as applicable. If needed, you may receive a follow-up phone call or e-mail to confirm or clarify information.
