2.0 GENERAL NOTES
All DID numbers contain the prefix “FAA-OES-” followed by a four-digit code starting with one letter followed by three digits. The letters in the code are “A” for management, “T” for test, “L” for logistics, and “M” for training. CDRL numbers all use the same DID four digit-code.
All days are calendar days unless stated otherwise. All submitted electronic material must be in designated Microsoft Office Suite 2016 or newer unless alternative software file format is approved by the Government.
All deliverables must be submitted with a separate cover letter as designated in Block 15 of the CDRL. The cover letter must be on the Contractor’s official company stationery, and it must be signed by the Contractor’s designated Program Manager. All deliverables must also be submitted with a cover page. The cover page must contain the document control number in the upper right-hand corner. In the center of the page, these words must appear in the following format:

The CDRL form uses FAA Template No. 59. There is an instruction sheet (3.0) below that describes the contents of each block. It also lists the ACRONYMS or “CODE” used in the appropriate blocks. The DIDs use FAA Template No. 60.
There are no special instructions for DIDs.
3.0 CONTRACT DATA REQUIREMENTS LIST DESCRIPTIONS
Item A Contract Line Item No.: Contract Line Item Number (CLIN) associated with this Contract Data Requirements List (CDRL). Not Used.
Item B Exhibit: Exhibit identifier for this CDRL item.
Item C Category: This block provides the category of the item to be provided by the contractor. Other categories besides the Technical Data Package or Technical Manual can be Provisioning, Configuration Management, Test Plans, etc.
Item D System/Item: System, item, project designator or name, or title of services being acquired that the data will support.
Item E Contract/PR No.: Procurement Instrument Identification Number (PIIN) (i.e., the contract number, the Purchase Request (PR) number, the Screening Information Request (SIR) number, or other appropriate designators.
Item F Contractor: Contractor’s name.
Item G Prepared by: Preparer’s organization responsible for preparing the CDRL.
Item H Date: Date the form was prepared.
Item I Approved by: Office responsible for approving the CDRL. The approving official’s signature appears on the last page of exhibit.
Item J Date: Date the form was approved.

Block 1 Data Item No. : The four-character Exhibit Line Item Number (ELIN). The first position consists of the exhibit identifier and the remaining positions are assigned sequentially.
Block 2 Title of Data Item: The title of the Data Item Description (DID) cited in item 4.
Block 3 Subtitle: Further identification of the data item to supplement the title, if required.
Block 4 Authority (Data Acquisition Document No.) – DID identification number where detailed preparation instructions are provided.
Block 5 Contract Reference: The specific paragraph number of the applicable contractual document that contains the tasking that generates a requirement for the data item. Normally this will be a Section C paragraph.
Block 6 Requiring Office: The technical office having responsibility for ensuring the technical adequacy of the data.
Block 7 Letter Required: Letter of Transmittal required.
Block 8 Approval Code: Identification, by entry of an “A” of a requirement for advance written approval prior to final distribution of an item of data. An “A” in this item indicates that one or more draft submittals are required. Item 14 will show length of turn-around time for the contractor to resubmit the data after Government approval/disapproval has been issued. The turn-around time for Government approval/disapproval will be 30 business days after receipt of submittal. An “R” indicates Review.
Block 9: Distribution Statement Required: For technical data, specify requirement for Contractor to mark the appropriate distribution statement on the data.

Block 11 As of Date: The date for cutting off collection of the data. If the data is to be submitted multiple times, a numeral will be entered to indicate the number of calendar days prior to the end of the reporting period, established in item 10, that data collection will be cut off. For example, “15” would place the “as of date” for the data at 15 days before the end of the
J2-4 month, quarter, or year, depending upon the frequency established in item 10: a “0 or blank” would place the “as of” date at the end of each month, quarter, etc.
Block 12 Date of First Submission: Due date for initial data submission. This is normally the postage date or date the data is delivered to the Contracting Officer or keyed to a specific event or milestone, using a code from Table 2.

The “N” is assigned a value indicating the number of days, e.g., 30 DACA. Unless otherwise stated, “days” refers calendar days.
Block 13 Date of Subsequent Submission: Due date(s) for subsequent data submission(s) if data is to be submitted more than once. If submittal is constrained by a specific event or milestone, the constraint will be given using a code from Table 2.
Block 14 Remarks: Contains all pertinent information not specified elsewhere and any amplification of other items on this format (e.g., DID tailoring, approval criteria and authority, inspection and acceptance clarification, distributions statements, specific submission instructions, explanation of reproducible copy and delivery requirements, alternative medium for delivery of the data item, etc.). Unless otherwise specified, all documentation must be in Microsoft Office 2016 or Adobe Acrobat 6.0.
Block 15 Distribution: Addressees and the number of initial and final copies (regular or reproducible) to be provided to each. FAA facility designator and/or office symbols/codes may be used; however, an explanation of these will be provided in the Addressee Listing. When reproducible copies (e.g., magnetic diskettes/tape, vellum, negative, etc.) are required, an explanation will be provided in item 14. If deferred delivery is required, amplification will be given in item 14.

For all CDRL items, the initial delivery and all revised deliveries must be in electronic. Once the contractor has received final approval of the CDRL item, they must deliver a final paper copy (FP) to the Contracting Officers Representative (COR). Deliveries to the Contracting Officer (CO) must be a Letter of Transmittal Only (LTO).

Master Contract Management Plan (MCMP) FAA-OES-A001
3. Description/Purpose
3.1 This data item describes the contractor’s management organization, assignment of functions, duties, and responsibilities to include interrelationships between the prime Contractor, major subcontractor(s) and the contracting activity, management procedures and reporting requirements for the conduct on contractually imposed tasks, projects, or programs.
3.2 The MCMP will provide details of the specific techniques, tasks, and procedures to be used for monitoring contract management and performance, configuration control, data management, production management, and cost control.
3.3 The MCMP provides the contracting activity a basis for reviewing and evaluating contractor performance, and for determining contractual compliance.

7.1 This DID must be the basis for the Contractor’s management approach to ensuring adherence to program technical, schedule, and cost objectives.

10. Preparation Instructions
10.1 General Instructions.
a. Format. Electronic deliveries must be in .docx or .xlsx format and must be compatible with MS Office unless alternative software is approved by the Government. Contractor format is acceptable.
b. Cover Page. The MPMP must contain a cover page as described in Section 2.0 General Notes of Attachment J2.
c. Descriptive Material. As may be needed to clarify or explain matter in the text, the plan may include descriptive material, sketches, drawings, photographs, table forms, graphs, worksheets, charts, etc. The MCMP must be delivered to the FAA in both paper and electronic media using MS Office products unless otherwise approved by the Government.
d. Table of contents, document size and acronyms list. The MCMP must be on standard size paper, and it must not exceed 40 double sided, single spaced pages. The pages must be sequentially numbered and bound together. The plan must include a table of contents and a definition list of all acronyms used.

10.2 Content Requirements. The MCMP must cover the organizational structure, program management methodology, personnel, resource management, program planning and scheduling tools and techniques, production planning and management, subcontractor management, and reports as described herein. As a minimum, other subjects to be included are described in the following sections:
a. Purpose. This paragraph must describe the purpose of the MCMP in terms of its relationship to the management of the project and performing the contract tasks outlined in the Statement of Work (SOW).
b. Scope. This section must contain programmatic and technical background on the system; an overview of the Contractor’s approach to program technical, schedule, cost, and risk control; authority of the program manager; and the relationship of the MCMP to the other programmatic policies, procedures, and planning documents.
c. Contract Management. This section must provide information on the Contractor’s management organization, internal management policies and procedures, an overall integrated program schedule, relationships with Government personnel and agencies, and roles and responsibilities of management entities within the organization.
d. Management Organization. Present a company organizational chart(s) and sufficient supplemental narrative to fully describe all organizational levels and activities. This chart must be hierarchical in nature and should clearly delineate all major area responsibilities and management positions. Provide a similar chart of the program organization to be used in performance of the contract. If the project includes major subcontractors, provide organizational and functional information about the subcontractor.
e. Roles and Responsibilities. Discuss the authority of all responsible management positions identified by the organizational description. This description must include the role of the project manager to direct, control, and commit resources to adequately fulfill their responsibilities.
f. Personnel. The plan must list the personnel staffing of the contractor’s management office that directs the contractual tasks, projects, and programs and assist in completing the contract. The list must include the identity, background, and work experience of the personnel assigned to fill these positions. As a minimum, the experience of personnel listed must be IAW the requirements in Attachment J5.
g. Staffing. The plan must also describe the staffing required for proper accomplishment of each contractual task by skill type, level, and number of personnel. This description must include a chart depicting the projected variations in staffing levels planned for each project phase.
h. Policies and Procedures. Describe internal policies and procedures to be used in managing the contract.
i. Relationships. Describe the working relationships the Contractor will establish with the Government (FAA) and any subcontractors supporting the procurement of the equipment.
j. Subcontractor management. Describe the approach to managing subcontractor activities. Include in the approach the organizational relationship maintained between the prime and subcontractors and methods of requirement flow-down and activity progress reporting back to the prime Contractor. Describe subcontractor and supplier rating systems used to select subcontractors and describe the approach to maintaining quality from products produced by subcontractors.

k. Schedule Management. Provide a detailed description of how the contractor will implement a fully integrated, defined, planning and controlled scheduling system. The description must include discussion of interrelationship of tasks, and tracking criticality of the tasks. If subcontractor(s) are used, similar information must be presented and must include a discussion on how contractor and subcontractor schedules must be integrated and updated.
l. Resource Planning. Provide a detailed description of how the Contractor will allocate and plan resources to meet the delivery requirements of the project. Discuss any resource planning tools used for this purpose.
m. Risk Management. Describe the internal approach and method for the identification, assessment, and mitigation of program risks. This approach should include provision for identifying risk areas, assessing risk factors, assigning appropriate resources to reduce risk factors, identifying, and analyzing alternative actions available, identifying the most promising alternatives, and planning for implementation of risk reduction. Describe contractor procedures to develop a plan of action to mitigate risk.
n. Data Management. Describe the organization, procedures, and tools to be used to ensure that all data deliverables required by the contract are made in a timely manner. Identify the individual responsible for integrating and maintaining the total data management effort. This effort must involve monitoring, reporting, status accounting, and cross matrixing (e.g., Government change requirements versus implemented changes) of all changes to, additions of, or deletions of CDRL contents. The Contractor’s procedures for controlling the generation, receipt, approval, storage, and delivery of subcontractor data as well as its inclusion in status accounting must be also described.
o. Earned Value Management System Plan. Describe the organization, methods, procedures, and controls used to plan, schedule, budget, monitor, manage, and report cost, schedule, and technical status applicable to the contract. Describe how the Contractor’s EVMS complies with the industry guidelines delineated in Electronic Industries Alliance EIA-748 revision at the time of contract award. Identify the individual responsible for integrating and maintaining the total earned value management system effort.
This effort must involve effective integration of technical, cost, and schedule information about contract performance. The plan must:
1. Describe the EVMS the contractor intends to use in performance of the contract, and how the proposed EVMS complies with the EVMS guidelines in EIA-748
2. Distinguish between the contractor's existing management system and modifications proposed to meet the EVMS guidelines, it necessary
3. Describe the management system and its application in terms of the EVMS guidelines
4. Describe the proposed procedure for administration of the EVMS guidelines as applied to subcontractors
5. Describe the process the contractor will use to determine subcontractor compliance with EIA-748

6. Identify the subcontractors, or major subcontracted effort if subcontractors have not been selected, to whom the EVMS requirements will apply
7. Identify a schedule that provides a timetable of events leading up to Government certification of the EVMS
8. Provide information and assistance as required by the Contracting Officer to support review of the plan.
p. Quality Control, Performance Evaluation, and Contractual Compliance. This section must discuss Contractor and major subcontractor(s) methods and concepts for continuous quality control, performance evaluation, and contractual compliance.
q. Reviews and Reporting. Describe plans for all formal and informal reviews and reporting to the Government.
r. Systemic Improvement. Describe the approach to systemic improvement of products. Include in the discussion the approach to supplying quality products and making continuous improvement to critical processes controlled by the organization. Describe the tools and techniques which are used or planned for use on this contract. Include any discussion of relevant experience in the area of systemic improvement.
s. Production Management. This section must discuss Contractor and major subcontractor(s) methods and concepts for employing facilities, tooling, and manpower resources to produce the system/equipment.
t. Manufacturing Methods and Production Flow. Provide a production flow utilizing a goes-into chart, tree chart or equivalent to detail the process for manufacturing and assembly in terms of key operations or assembly points showing individual and total lead times from the procurement of raw material to delivery of the end item.
u. Capability and Capacity. This section describes the data and information used to determine the Contractor and subcontractor(s) capability and capacity to meet schedule requirements. The Contractor must provide data indicating ability to meet the production rate and quantities required for the system. Manufacturing risks and the systems used to track suppliers' progress and to detect/correct potential delivery problems must also be addressed. Provide a list of all subcontractors and major vendors indicating the services/ materials to be supplied by each.

3. Description/Purpose
3.1 The Quality System Plan (QSP) describes the contractor’s specific implementation of the quality management system needed to comply with FAA contract requirements. The QSP describes quality objectives, procedures, and processes.
3.2 This QSP provides the Government contracting activity a basis for assessment of the quality system and evidence of the contractor’s intent to comply with the contract quality requirements.

10. Preparation Instructions
10.1 General Instructions:
a. Format. Electronic deliveries must be in .docx or .xlsx format and must be compatible with MS Office unless alternative software is approved by the Government. Contractor format is acceptable.
b. Cover Page. The QSP must contain a cover page as described in Section 2.0 General Notes of Attachment J2.
c. Descriptive Material. As may be needed to clarify or explain matter in the text, the plan may include descriptive material, sketches, drawings, photographs, table forms, graphs, worksheets, charts, etc. The QSP must be delivered to the FAA in both paper and electronic media using MS Office products unless otherwise approved by the Government.
d. Table of contents, document size and acronyms list. The QSP must be on standard size paper, and it must not exceed 40 double sided, single spaced pages. The pages must be sequentially numbered and bound together. The plan must include a table of contents and a definition list of all acronyms used.

10.2 Content Requirements: Content Requirements begin below.
a. Documentation (General): The contractor Quality Management System documentation must be defined.
In addition, the contractor must have documented procedures to address; Purchasing; Production and Service; Customer Property; Preservation of Product, to include Handling, Storage, and Packaging; and Control of Monitoring and Measuring Devices.
b. Control of Records. The contractor must control records for the documented procedures as additionally required by this DID. The QSP must address the availability of records for evaluation by the customer or the customer’s representative. The contractor must establish and maintain records, which provide evidence that the product has been inspected and/or tested. These records must show clearly whether the product has passed or failed the inspections and/or tests according to defined acceptance criteria.
Where the product fails to pass any inspection and/or test, the procedures for control of nonconforming product must apply. Records must identify the inspection authority responsible for the release of product.
c. Responsibility, Authority, and Communications. The QSP must describe the management structure responsible for operation of the QMS. In addition to defining the responsibilities and authorities within the organization, top management must ensure that personnel who verify work affecting quality have the organizational freedom and authority from persons directly responsible for developing products or executing process within the project to:
1. Initiate action to prevent the occurrence of any nonconformity relating to product, process, and quality system;
2. Identify and record any problems relating to the product, process, and quality system;
3. Initiate, recommend, or provide solutions through designated channels:
4. Verify the implementation of solutions;
d. Control further processing, delivery, or installation of nonconforming product until the deficiency or unsatisfactory condition has been corrected.
e. Customer Communication. The QSP must address implementation of effective communications with customer quality assurance representative.
f. Purchasing. The contractor must establish and maintain documented procedures to assure that purchased products conform to specified requirements. The contractor must ensure that incoming product is inspected or verified on receipt as necessary to assure conformance to specified requirements. The customer must have access to all contractor facilities necessary to determine product conformance. Verification by the customer must not absolve the contractor of responsibility to provide acceptable product.
g. Control of Production and Service Provision. The contractor must plan and control the production processes that directly affect product quality and must ensure that such processes are carried out under controlled conditions. The contractor must establish and maintain documented procedures to assure that such controlled conditions meet specified requirements.
h. Identification and Traceability. The QSP must identify processes for identification and traceability.

i. Customer Property. The contractor must establish and maintain documented procedures to assure that customer property is properly controlled.
j. Preservation of Product. The contractor must establish and maintain documented procedures for handling and storage of the product. The contractor must provide methods for packaging and handling deliverable end-item(s) to prevent damage or deterioration. The contractor must use designated storage areas or stock rooms for deliverable end-item(s) to prevent damage or deterioration, pending use or delivery. The contractor must control packing, packaging, and marking processes for deliverable end-item(s) as necessary to ensure conformance to specified requirements.
k. Control of Monitoring and Measuring Devices. The contractor must establish and maintain documented procedures to assure monitoring and measuring devices are correctly maintained and calibrated as needed to assure accurate measurement and provide correct evidence of product conformity. The QSP must address the standard used for calibration of test and measuring devices that is traceable to national or international measurement standards.
l. Monitoring and Measurement of Processes. The contractor must provide customer access to their premises. The customer must have access to all contractor facilities necessary to evaluate the ability of processes to achieve planned results.
m. Monitoring and Measurement of Product. In-process inspection and/or testing must be performed as necessary to assure conformance to specified requirements. Persons performing such inspection/test must have authority to prevent any nonconformity related to the product. The contractor must perform final inspection and/or test as necessary to substantiate product conformance to contract requirements. The contractor must verify that conduct of such final inspection/test complies with approved procedures; that hardware configuration status is properly identified; that discrepancies/open items are properly documented; and (if necessary) that corrective action is properly implemented. Results of final inspection/test must be conclusive as necessary for government acceptance. The contractor must provide customer access to contractor and suppliers premises. The customer must have access to all facilities necessary to determine product conformance. Verification by the customer must not absolve the contractor of responsibility to provide acceptable product.
n. Control of Nonconforming Product. The contractor must physically segregate nonconforming material to prevent misuse.
o. Corrective Action. The contractor must provide a written response to the customer quality assurance representative for any corrective action resulting from customer complaint.
p. Preventive Action. The contractor must review feedback from the customer‘s quality assurance representative during the preventive action process.

a. Documentation (Specific). The health and safety plan should contain as a minimum the following major elements:
1. Management commitment and assignment of responsibilities
2. Safety communications
3. Hazard assessment and control
4. Accident/exposure investigation
5. Safety planning, rules & work procedures
6. Safety and health training and instruction
7. Record keeping and documentation
8. Emergency contact list (e.g. local first-responders, FAA points of contact, etc.) and map/directions to the nearest emergency care center.
9. Specific OSHA (29 CFR 1910 and 1926) safety standards applicable for the work involved (e.g.
trenching/shoring, fall protection, lock-out/tag-out, portable hand and power tools, cold and heat stress, personal protection equipment (PPE), emergency action plan, etc.)
10. Any chemicals/hazardous substances to be brought on-site must have a Manufacturers Safety Data Sheet (MSDS) and be included for review by the facility’s Sector Environmental Compliance Manager
(SECM).
11. Unanticipated chemicals to be brought on-site must first be coordinated with the facility’s SECM no less than 24 hours of bringing them on-site
12. Expert Certification for towers.
b. Site Specific Information: When project site and intended scope of work are known, the emergency contact list and map/direction of the nearest emergency care center should be provided along with the generic safety plan.

7.1 This Data Item Description (DID) is applicable to all contracts for systems/equipment, related items and services procured.
7.2 This Data Item Description (DID) contains the format and content preparation instructions for the WBS to be submitted by the contractor, as required, by SOW paragraph C.4.2.2.
a. Format. Electronic deliveries must be compatible with MS Office unless alternative software is approved by the Government.
b. Cover Page. The CWBS must contain a cover page as described in Section 2.0 General Notes of Attachment J2.
c. Purpose. The complete work breakdown structure (WBS) will serve as a basis for program and technical planning, scheduling, cost estimating, resource allocations, performance management, configuration management, and status reporting. The CWBS must identify all subcontracted elements or tasks and must include an index and dictionary (CDRL-A006).
d. Level of Detail. The Contractor must extend the CWBS specified in the contract to a level sufficient to identify all high-cost or high-risk elements and any elements of special interest to management. The CWBS must be extended to the necessary level of detail to support development of a logical and efficient sequence of tasks designed to accomplish the effort described in the Contract. The CWBS must include the contractor’s extension to at a minimum include control accounts.

a. Line Number. Enter line number for each CWBS. Line numbers should be sequential starting with 1.
b. CWBS Element. Enter the title of the CWBS element (using the specific name or nomenclature, when applicable), intended to reflect the level. Level 1 is the total contract. Level 2, 3, etc., are successively lower levels of the program.
c. CWBS Code. Enter the CWBS code, if applicable.
d. Contract line item(s). Enter the number of the contract line item(s) which is (are) associated with the CWBS element, if applicable.
e. Statement of work (SOW) paragraph number(s). Enter the applicable paragraph number(s) from the SOW, if applicable.
