About this file
This is the NAVSUP Publication 538 Sixth Revision MHE (Materials Handling Equipment) Inspection Form, a standardized template used for daily pre- and post-operation inspections of powered materials handling equipment by operators.
The form documents inspection results across 24 mandatory inspection categories covering critical equipment components and safety systems, including tires and rims, engine oil and fluid levels, radiator coolant, battery, access covers, fuel system, engine noises, lights, horn, hoist, tilt and side shift, transmission/clutch, directional controls, brake system, gauges/meters, fire extinguisher, operator restraint systems, forks, fork positioning locks and stops, fork safety chains, overhead guard and load backrest, ground straps and conductive tires/wheels, structural cracks, and mandatory markings. Each inspection item contains columns for marking satisfactory (SAT) or unsatisfactory (UNSAT) conditions at both the start and finish of each shift, along with space for shift hour-meter readings. The reverse side of the form contains detailed inspection criteria procedures for each of the 24 items, providing specific guidance on what constitutes acceptable or defective conditions and safety requirements. The form requires signatures from both the initial operator and last operator to ensure accountability for equipment condition throughout each operational period. If defects are identified, equipment must be removed from service, the supervisor notified, and the form retained until repairs are completed.
