About this file
This is a Quality Assurance Surveillance Plan (QASP) for the NAVSUP FLCJ Gulfport Material Handling Equipment (MHE) Maintenance Contract. The QASP establishes the Government's methodology for monitoring and evaluating contractor performance in maintaining material handling equipment, specifically forklifts, in accordance with the Performance Work Statement, OEM standards, and industry best practices. The plan defines surveillance procedures, performance standards, and acceptable quality levels to ensure services meet contractual requirements and support mission readiness.
The QASP identifies seven key performance objectives with measurable standards and corresponding quality acceptance levels: Preventive Maintenance must be completed on-time 95% of the time per OEM standards; Corrective Maintenance repairs must be completed within OEM time plus two hours of troubleshooting with 95% compliance; Response Time requires 100% compliance in responding to service calls within one business day; Parts Standards mandate 100% compliance using new or like-new OEM-specification parts; Safety/Environmental Compliance requires 100% adherence to Navy, OSHA, and environmental regulations; Recordkeeping must achieve 100% accuracy and completeness; and Equipment Availability must maintain at least 90% forklift fleet availability. Surveillance methods include random inspections, periodic inspections, 100% inspection of critical safety tasks, customer feedback, and records review. The Government will enforce performance through incentives—recognizing satisfactory performance in future past performance evaluations—and remedies including corrective action at no cost to the Government, cure notices, payment withholding, and potential termination for default. The Contracting Officer's Representative documents all surveillance results and immediately reports safety or mission-readiness deficiencies to the Contracting Officer.
