1.0 INTRODUCTION
Tetra Tech EC, Inc. (TtEC) has been contracted by the United States (U.S.) Department of the Navy (Navy), Naval Facilities Engineering Command (NAVFAC) Mid-Atlantic under NAVFAC Atlantic Remedial Action Contract N62470-13-D-8007, Contract Task Order JM02 to perform culvert and causeway repairs at Site 3, Marine Corps Recruit Depot (MCRD) Parris Island, located on Parris Island, South Carolina. A Site Location and Vicinity Map for Site 3 is included as Figure
1. A Site Overview showing the two culvert areas is included as Figure 2. This Letter Completion Report has been prepared to document the field activities implemented to address newly formed sinkholes and other void areas at the north and south culvert areas of the causeway. The letter includes background information, a summary of field activities, a health and safety summary, a quality control summary, and supporting attachments.
2.0 BACKGROUND
In December 2018, new sinkholes were discovered at the causeway. These sinkholes were significant enough to require closing the causeway road, which is one of only two access roads to MCRD Parris Island. A preliminary geophysical survey of the culvert areas of the causeway was completed in January/February 2019. The findings were included in a letter from MCRD Parris Island to the U.S. Environmental Protection Agency and South Carolina Department of Health and Environmental Control, dated February 13, 2019 (MCRD 2019). The anomalies identified during the preliminary geophysical survey are shown on Figures 3 and 4. To address these anomalies, the Navy requested that TtEC perform immediate sinkhole repairs. A Letter Work Plan, outlining the operational approach to address the sinkholes and void areas by injecting structural polyurethane foam grout in the subsurface of the causeway at the north and south culvert areas, was prepared and submitted on February 21, 2019 (TtEC 2019a).
3.0 SUMMARY OF FIELD ACTIVITIES
The following field activities were implemented by TtEC and its subcontractors during March 2019 to complete the immediate sinkhole repairs at Site 3: mobilization, site preparation, injection activities, site restoration, waste management, and demobilization.
3.1 MOBILIZATION
Project personnel, equipment, materials, and temporary facilities were mobilized to MCRD Parris Island as needed to execute the field activities. Temporary facilities included portable toilets, a portable handwash station, and a dumpster for general trash service. The temporary facilities were installed in the support areas shown on Figures 3 and 4. Mobilization activities were completed on March 4 and 5, 2019.
TtEC followed the MCRD Parris Island Ground Penetrating Activity Standard Operating Procedure (SOP) during the execution of the immediate sinkhole repairs. The Ground Penetrating Activity Permit Request Form was completed and submitted to MCRD Parris Island.
In accordance with the SOP, South Carolina 811 was notified prior to submitting the request form so that the ticket numbers could be included. In addition, a third-party utility locator, Ground Penetrating Radar Systems, was utilized to locate and identify subsurface utilities in the work areas. A copy of the third-party utility locate report is included as Attachment 1.
3.2 SITE PREPARATION
Following mobilization, site preparation activities commenced on March 5, 2019. TtEC established work areas and support areas as required to complete the project activities. The causeway road remained closed during the sinkhole repair activities. To the extent possible, TtEC utilized the paved portion of the causeway for support areas to limit disturbance to grassy areas.
Two support areas were established, one on the land side of each culvert area, as shown on Figures 3 and 4. Vehicles and equipment accessed the culvert areas from the land side of each culvert area and were not permitted to travel over known sinkhole areas. The support areas were used to stage material and equipment during execution of the field activities. A poly-lined, bermed and covered staging area was established at the northwest end of the causeway for storage of bulk containers of injection material.
The proposed injection grid was marked out at both culvert areas and was adjusted slightly as needed to account for underground utilities and guard rails. In the north culvert area, one row of injection points was eliminated due the presence of an underground communication line. In the south culvert area, 10 injection points were added on the north side of the road to ensure all identified anomalies and surface depressions were captured by the injection grid. The as-built injection grid was surveyed to document the injection points. The survey is included in Attachment 2 and the injection points are shown on Figures 3 and 4.
As a precautionary measure, TtEC installed floating booms at both sets of culvert pipes on both ends of the pipes to catch injected material that could pass through the open lifting holes of the pipes and into the pond or marsh. The injection material is a foam product that floats on top of the water, making it easy to identify and recover using the boom to pull it to shore or using a pool skimmer. Injection material was observed floating on the water surface while injecting at the 6-foot and 9-foot depth intervals in the north culvert area, and while injecting at the 6-foot depth interval in the south culvert area. All observed injection material was captured by the booms, recovered using pool skimmers, and placed in a dumpster for disposal as general construction debris.
3.3 INJECTION ACTIVITIES
Following completion of the site setup activities, injection activities commenced on March 12, 2019. UMA Geotechnical Construction was subcontracted to provide the injection services.
Structural polyurethane foam grout was injected into the subsurface of the causeway at the north and south culvert locations. The injection material used was a two-part mixture consisting of BaySystems 486Star-4# Resin and Mondur MR Isocyanate. Product data for the injection material used is included in Attachment 3. The horizontal and vertical extents of the injection areas are shown on Figures 3 and 4. For the areas shaded blue, sacrificial injection rods were driven on a 4-foot by 4-foot horizontal grid at 3-foot and 6-foot vertical intervals to achieve the desired 6-foot treatment depth. For the areas shaded yellow, sacrificial injection rods were driven on a 4-foot by 4-foot horizontal grid at 3-foot, 6-foot, and 9-foot vertical intervals to achieve the desired 9-foot treatment depth. The structural polyurethane foam grout was then injected into the subsurface through the sacrificial rods. During injection activities, the injection volume was monitored and documented. The roadway surface was monitored for movement using a laser level with receivers surrounding the injection point. Injection was stopped at each injection point when movement of 1 to 2 millimeters was detected at the surface, when material was observed daylighting at the ground surface, or if high pressure was encountered at the injection gun. A total of 8,445 pounds of material was injected into the ground at the north culvert area, and a total of 8,727 pounds of material was injected into the ground at the south culvert area. The injection footprints and injection quantities are shown on Figures 3 and 4. Tables presenting the injection quantity at each point and depth are included as Attachment 4. Injection activities were completed on March 28, 2019.
3.4 SITE RESTORATION
Following injection activities, TtEC broke down the equipment, collected excess materials, and performed general cleanup and housekeeping of the work areas and support areas. The aboveground portions of the injection rods and surficial foam were removed and placed in a dumpster for disposal as general construction debris. The empty containers of injection material and the boom placed at the culvert pipes during site preparation were removed and taken off site for future reuse by the injection subcontractor. The remaining portion of the injection rods were driven below the ground surface, and, in asphalt areas, they were patched with non-shrink grout or asphalt patch. Surface depressions in grassy areas were filled with crusher run gravel, topsoil, and centipede grass sod. Surface depressions in the jogging path were filled with crusher run gravel. Settlement/erosion areas near the culvert headwalls were filled using rip rap and an underlying layer of non-woven geotextile. Site restoration activities were completed on March 29, 2019.
3.5 WASTE MANAGEMENT
The only wastes generated during the field activities were general municipal waste and construction debris. These were collected in a dumpster and shipped to a local landfill for disposal.
3.6 DEMOBILIZATION
Upon completing the activities described above and the final walkthrough described in Section 5.0, TtEC initiated demobilization activities. Site personnel, temporary facilities, equipment, and excess materials were removed from MCRD Parris Island. Demobilization was completed on March 29 and 30, 2019.
4.0 HEALTH AND SAFETY SUMMARY
Field activities conformed to the requirements and procedures included in the Final Accident Prevention Plan Culvert and Causeway Repairs at Site 3 (TtEC 2019b), submitted by TtEC to the Navy on February 11, 2019. Before beginning work, project personnel participated in orientation training as required by the Final Accident Prevention Plan. This training included a review of the site history and site-specific hazards. Safety meetings were conducted each day that work was performed on site. Applicable activity hazard analyses were reviewed and signed prior to initiating new tasks. During the sinkhole repair activities, TtEC and its subcontractors worked a total of 22 days and 1,463 hours with no recordable, first aid, lost-time, or vehicle incidents.
5.0 QUALITY CONTROL SUMMARY
The field activities were considered one definable feature of work, and the three phases of quality control (preparatory, initial, and follow-up) were utilized during the execution of the work. TtEC completed daily Contractor Production Reports and Contractor Quality Control Reports whenever on-site activities occurred. Copies of the daily reports are included in Attachment 5. At the completion of field activities, a final site walkthrough was conducted with the NAVFAC Construction Manager and the MCRD Parris Island Public Works Officer. No issues were noted during the walkthrough, and demobilization began the same day. Photographs were taken of the site to document progress before, during, and after fieldwork. A select number of photographs have been compiled into a photographic log included in Attachment 6.

Third-Party Utility Locate Report
Subsurface Investigation to Locate Underground
Utilities Prepared For: Tetra Tech EC, Inc
Prepared By:
Andrew Marques Project Manager-South Carolina
2/28/2019
February 28, 2019
Tetra Tech EC, Inc Attn: Louie McTall Site: 3rd BN Pond Road Parris Island, SC
We appreciate the opportunity to provide this report for our work completed on 2/28/19 at the above address in Parris Island, SC.
PURPOSE
The purpose of this project was to search for underground utilities within the project boundaries provided by the client. Area was to be scanned for any utilities that may interfere with injections to fill voids under asphalt.
EQUIPMENT
• 350 MHz GPR Antenna. The antenna is mounted in a stroller frame which rolls over the surface. The surface needs to be reasonably smooth and unobstructed in order to obtain readable scans. Obstructions such as curbs, landscaping, and vegetation will limit the feasibility of GPR. The data is displayed on a screen and marked in the field in real time. GPR works by sending pulses of energy into a material and recording the strength and the time required for the return of the reflected signal. Reflections are produced when the energy pulses enter into a material with different electrical properties from the material it left. The strength of the reflection is determined by the contrast in signal speed between the two materials. The total depth achieved can be as much as 8’ or more with this antenna but can vary widely depending on the conductivity of the materials. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. For more information, please visit: Link
• Electromagnetic Pipe Locator. The EM locator can detect the electromagnetic fields from live power or radio frequency signals. It can also be used in conjunction with a transmitter to connect directly to accessible, metallic pipes, risers, or tracer wires.
A current is sent through the pipe or tracer wire at a specific frequency and the resulting EM field can then be detected by the receiver. The receiver is moved over the surface without coming in contact with the ground so it is not affected by terrain. Depths provided should always be treated as estimates as their accuracy can be affected by multiple factors. Depths achieved can be as much as 20’ depending on the type of signal being traced or methods used. For more information, please visit: Link
PROCESS
Our process began with using the EM pipe locator to locate pipes or utilities throughout the scan area. We first sweep all areas with the receiver to detect live power or radio frequency signals followed by connecting to any visible risers or tracer wires that may be in the area provided that there is an exposed metallic surface. Locations and depths are painted or flagged on the surface. Depths cannot always be provided depending on the location method and can be prone to error.
Initial GPR scans were then collected in order to evaluate the data and calibrate the equipment. Based on these findings, a survey strategy is formed, typically consisting of scanning the entire area in a grid with 5’-10’ scan spacing in order to locate any potential utilities that were not found with the pipe locator. The GPR data is interpreted in real time and anomalies in the data are located and marked on the surface along with their depths using spray paint, pin flags, etc. Depths are dependent on the dielectric of the materials being scanned so depth accuracy can vary throughout a site. Relevant scan examples were saved and will be provided in this report.

LIMITATIONS
Please keep in mind that there are limitations to any subsurface investigation. The equipment may not achieve maximum effectiveness due to soil conditions, above ground obstructions, reinforced concrete, and a variety of other factors. No subsurface investigation or equipment can provide a complete image of what lies below. Our results should always be used in conjunction with as many methods as possible including consulting existing plans and drawings, exploratory excavation or potholing, visual inspection of above ground features, and utilization of services such as One Call/811.
At this site, our scans were limited by guard rails, marsh embankment and exposed voids in road.
FINDINGS
We found that the soil allowed for maximum GPR depth penetration of 6’ in most areas. We were able to locate communication lines and one unknown line in first area with the GPR or RD.
The following pages will provide photos and further explanation of our findings.

BaySystems 486Star PUR
Product Information
Polyurethane Foam Grout System
Product Description BaySystems 486Star polyurethanes are a family of two component high density thermoset polyurethane foam systems. They are prepared by mixing the BaySystems 486Star Resin with Mondur MR Isocyanate. The Isocyanate is commonly referred to as the “A” Component, while the Resin is known as the “B” component.
When properly combined in a 1:1 ratio by volume, BaySystems 486Star PUR systems are capable of expanding by up to 25 times their original volume in unconfined spaces.
BaySystems 486Star PUR systems are formulated to obtain various foam densities and with different reactivities for specific project applications. These applications may include:
Slab Jacking and concrete lifting Soil stabilization to increase its load bearing capacity Infrastructure and foundation repairs Void filling Bridge approaches
Unique Properties BaySystems 486Star PURs have a composition that allows for its direct application in wet or very damp regions while still maintaining good physical properties. Because it contains a hydrophobic additive, BaySystems 486Star will react correctly even when being injected directly into water. This property makes it ideal for lifting and stabilizing in areas with elevated soil moisture. Because of the resulting foam’s hydrophobic nature, the BaySystems 486Star PURs will resist erosion and weakening from water.
The low viscosity of the BaySystems 486Star components lets it penetrate into the soil easily.
As it expands, it compacts loose soil and displaces water without detrimental dilution to the resin or loss of physical properties in the cured polyurethane.
