About this file
This is a Past Performance Questionnaire template designed to evaluate contractor performance on federal contracts, specifically for the NAVSUP Fleet Logistics Center Pearl Harbor solicitation N0060426R0003 regarding packing, containerization, and local drayage services for Direct Procurement Method (DPM) personal property shipments.
The questionnaire is completed by past customers or agencies that have worked with a contractor and submitted directly to the contracting office (kyle.p.quintal.civ@us.navy.mil or fax 808-473-3524), not to the contractor being evaluated. Respondents assess contractor performance using a five-point rating scale: G (Good—superior accomplishment and high satisfaction), A (Acceptable—fully compliant with positive satisfaction), M (Marginal—accomplishes most requirements with marginal satisfaction), UA (Unacceptable—failure to accomplish requirements), and N (Neutral—performance not applicable). Evaluation areas include quality of performance (conformance to requirements and specifications, workmanship standards, rework instances, and innovative applications); schedule performance (timeliness of delivery and deliverables); and responsiveness (ability to resolve problems and quality standards). The form requires narrative rationale for all ratings, with mandatory justification for unsatisfactory ratings. Additional sections request identification of contractor strengths and weaknesses, likelihood of future award, awareness of similar contracted efforts, and referrals to other relevant points of contact. Respondents must sign and date the completed questionnaire.
