About this file

This is a Past Performance Questionnaire template designed to evaluate contractor performance history for federal contract award decisions. The questionnaire is completed by past customers or contracting offices and submitted to NAVSUP Fleet Logistics Center Pearl Harbor (FLC PH) for evaluation of contractors bidding on solicitation N0060425R4003 for packing, containerization, and local drayage services of Direct Procurement Method (DPM) personal property shipments for Department of Defense personnel.

The questionnaire assesses contractor performance across three primary categories: Quality of Performance, Schedule Performance, and Responsiveness. Quality evaluations cover overall satisfaction, conformance to contract requirements and specifications, standards of workmanship, instances of rework, and innovative applications. Schedule performance addresses timeliness of delivery and submittal of deliverables such as reports and invoicing. Responsiveness evaluates the contractor's ability to address concerns, resolve problems, and achieve systemic improvements, as well as product quality and overall technical, schedule, management, and cost performance. Respondents rate each criterion using a five-point scale (G=Good, A=Acceptable, M=Marginal, UA=Unacceptable, N=Neutral) with required narrative rationale, particularly for unsatisfactory ratings. The questionnaire also requests identification of contractor strengths and weaknesses, willingness to re-award, awareness of similar contracted efforts, and referrals to other potential respondents. Responses should be submitted electronically to kyle.p.quintal.civ@us.navy.mil or faxed to 808-473-3524 rather than returned to the referring contractor.
