About this file
This is a Past Performance Information Form (PPIF) template used by offerors to document relevant contract experience for evaluation purposes under solicitation N0001426R3002, Office of Naval Research Code 32 Ocean Battlespace Sensing Support.
The form requires offerors to submit detailed information for each past or present contract they identify as relevant, including company/division details, CAGE code, and Unique Entity Identifier Number. For each contract example, offerors must provide comprehensive contract specifics such as customer name and contact information, contract number, contract type, period of performance, initial and final contract values, completion dates, and their role (Prime or Subcontractor). The form mandates identification of three primary government points of contact: Program Manager/Contracting Officer Representative (COR), Procurement Contracting Officer, and Administrative Contracting Officer, with complete address, telephone, FAX, and email information. Offerors must then provide a detailed description of relevant work performed, explicitly correlating each example contract to the Performance Work Statement (PWS) tasks outlined in the solicitation by entering each PWS task and documenting how the submitted example includes comparable tasks, or stating "No Comparable Task" if no correlation exists. The form also requires offerors to address task complexity comparisons, identify key and non-key personnel proposed under the current requirement who performed on the example contract, and document any staffing turnover experienced with explanations of how vacancies were filled. Offerors should provide separate completed forms for each contract identified, with additional sheets attached as needed.
