About this file

This is a payment authorization memo issued by the City of Detroit Department of Housing & Revitalization (HRD), Single-Family Rehabilitation Program, on February 21, 2023. The memo authorizes payment to Harris Design Build, the contractor on the Senior Emergency Program project located at 3158 E. Forest, for completed work without homeowner signature(s). The HRD made multiple documented attempts to obtain final homeowner signatures through phone, email, and in-person contact but was unsuccessful due to lack of communication from the homeowner. The contractor has completed all work in accordance with project specifications as verified by the assigned Housing Compliance Inspector.

The memo serves as formal authorization for the contractor to receive partial payment for work satisfying assigned specifications despite the inability to secure homeowner approval. The payment authorization was issued by Faris Fakhouri, Chief of Housing Inspections, with notification copied to Heather Zygmontowicz (Chief Direct Services & Strategy Officer), Anna Pinter (Single Family Home Repair Director), and Diane Onuigbo (Program Manager). Questions or disputes regarding this payment authorization should be directed to HRDHomeRepair@detroitmi.gov or Fakhourif@detroitmi.gov.
