About this file
This is an Invitation for Bid (IFB) form from the State of Illinois for vendors submitting offers to state contracting opportunities. The document serves as the standard bidding form that vendors must complete and submit as part of their proposal response. The IFB requires bidders to declare whether they are claiming any applicable state preferences under the Illinois Procurement Code, such as the Illinois Business Preference or Domestic Product Preference. Bidders must identify specific items qualifying for claimed preferences and provide supporting documentation, including any required affidavits. The form also requires bidders to identify any exceptions to the standard terms and conditions by submitting a redlined version of the State's Standard Terms and Conditions, which the State may accept or reject at its discretion.
The document emphasizes that completion of all required documents, as referenced in solicitation section A.1, is mandatory, and failure to meet requirements will result in disqualification. Bidders are directed to reference the Standard Terms and Conditions available on the Illinois Central Procurement Office website before submitting their bids. An authorized representative of the bidding organization must sign and date the completed form to formally submit the bid to the State of Illinois. The form contains multiple checkboxes and text fields for bidders to indicate preferences, exceptions, and certifications required under the Illinois Procurement Code.
