About this file
This is an Invitation for Bid (IFB) offer form (B50776) submitted to the State of Illinois. The document serves as the bidder's formal submission in response to a state solicitation, requiring vendors to acknowledge their understanding of all solicitation requirements and confirm their commitment to comply with the terms outlined in the request for proposals. The form includes sections for bidders to identify themselves, declare any claims for state-provided business preferences, and note any exceptions to standard contract terms and conditions. Bidders are directed to reference the Illinois Procurement Code and the state's standard terms and conditions available through the Central Procurement Office website.
The form requires bidders to complete all mandatory documentation as specified in the solicitation's Section A.1 and to submit any redlined versions of standard terms and conditions if exceptions are taken. Bidders may claim Illinois Business Preferences or Domestic Product Preferences, each requiring specific supporting documentation or notation. The bidder certifies through an authorized representative's signature that the submission constitutes a binding offer to perform in full compliance with the solicitation requirements, subject to the State of Illinois's acceptance.
