May 7, 2015 ID08150033 Medical Treatment Facility (MTF) Operations & Maintenance Amendment #4 Questions/Answers

1. Question: We are currently on the IDIQ contract with GSA to provide O&M services for AFMSA.

We have six sites. Do I send out the Past Performance Questionnaire to each site? Or do I send it to

GSA?

ANSWER: Past Performance Questionnaires must be filled out by a reference point of contact who can speak to the work that was performed on site. Sending the Past Performance Questionnaire to the Contracting Officer Representative (COR) at the applicable site would satisfy this requirement.

2. Question: We were on the previous contract vehicle with GSA to provide the same services to AFMSA. Can we use those sites as past performance since they were within the five year window?

ANSWER: Yes.

3. Question: Do we put our company letterhead on the Past Performance Questionnaire?

ANSWER: The Past Performance Questionnaire does not require company letterhead.

4. Question: Solicitation Page 33, Factor 3: Key Personnel Experience; PWS Page 3, Paragraph 1.6.1, Contractor Personnel Discussion: The Solicitation states the key personnel are the Program Manager, Project Manager, and Project Maintenance Manager. The PWS states that the key personnel are the Program Manager, Project Manager, Contract Maintenance Manager Clinical Business Occupancy, and Contract Maintenance Manager Health Care Occupancy.

Which document provides the correct key personnel?

ANSWER: At the IDIQ level, key personnel are comprised of an IDIQ Program Manager and an IDIQ Project Manager. At the Task Order level, key personnel is comprised is of Contract Maintenance Manager Clinical Business Occupancy (CMMC) or Contract Maintenance Manager Health Care Occupancy (CMMH) which is identified in the Task Order PWS.

5. Question: Solicitation Page 33, Factor 3: Key Personnel Experience; PWS Pages 35, Paragraph 1.6.1.11.6.1.4 Discussion: The Solicitation key personnel qualification requirements contradicts the contractor personnel qualification requirements provided in the PWS.

Should offerors follow the Solicitation key personnel qualification requirements or the PWS contractor personnel qualification requirements?

ANSWER: Offerors should use the PWS and Solicitation in providing a response to key personnel that includes the qualifications of the PWS and meets the evaluation criteria written in the Solicitation. The

PWS key personnel qualifications are minimum requirements identified for the positions. The Solicitation evaluation criteria identifies the factors that will be evaluated.

6. Question: Amendment 002, Page 1, Question 2.

Should at least one facility be located 1,000 miles from at least one of the other three facilities, or 1,000 miles away from all three remaining facilities?

ANSWER: At least one facility be located 1,000 miles from at least one of the other three facilities.

7. Question: Solicitation Page 33, Factor 3: Key Personnel Experience Clarification: Please confirm that offerors must provide a Contract Maintenance Manager/ Project Maintenance Manager resume for each of the seven locations.

ANSWER: At the IDIQ level, Offerors should provide experience/resume for each the IDIQ Program Manager and IDIQ Project Manager. At the Task Order level,Offerors should provide experience/resume for each site a Contract Maintenance Manager Clinical Business Occupancy (CMMC) or Contract Maintenance Manager Health Care Occupancy (CMMH) which is identified in the Task Order PWS.

8. Question: Amendment 002, Question 6 Discussion: The response to Question states that the Task Order Proposal Instructions are in Attachment 4. Attachment 4 is the Fairchild AFB Site Specific PWS.

Clarification: Please provide the task order proposal instructions.

ANSWER: The Task Order Proposal Instructions are Attachment 4 Ordering Procedures Task Order of the solicitation. Fairchild AFB is Attachment 4 under Amendment 1. The document, Task Order Proposal Instructions, is posted under “Solicitation 1” in FBO and is attached with this Amendment as well.

9. Question: Attachment 3, Page 7, Paragraph 1.9.1, Elmendorf AFB Site Specific Performance Work Statement

Clarification: Please confirm the limit of liability for all labor and parts, materials and supplies for individual unscheduled maintenance actions is $5000.00 or less.

ANSWER: Yes, the limit of liability for all labor and parts, materials and supplies for individual unscheduled maintenance actions is $5,000.00 or less.

10. Question: Attachment 6, Tinker AFB Site Specific Performance Work Statement Discussion: All site specific PWS’s have a section defining unscheduled maintenance performance which includes a limit of liability for all labor and parts, materials and supplies for individual unscheduled maintenance actions. For example paragraph 1.5.1 McConnell AFB indicates $500.00 or less. This language and limit is not addressed in the Tinker Site Specific PWS.

Clarification: Please clarify this requirement for Tinker AFB.

ANSWER: For Tinker AFB, the limit of liability for all labor and parts, materials and supplies for individual unscheduled maintenance actions is $500.00 or less. The Unscheduled Maintenance Performance language has been added to the Tinker Site Specific PWS and is attached with this Amendment.

11. Question: Page 31, 2. Addendum to 52.2121 Instruction to Offerors, Paragraph 2

Discussion: Solicitation states “Offerors must clearly label their IDIQ proposal as well as their subsequent task order proposals that they are interested in for each of the seven sites.”

Clarification: Please clarify whether the seven task orders must be part of our response or subsequent submittals.

ANSWER: Yes, the seven task orders must be part of the Proposal for this Solicitation.

12. Question: Amendment 3 modified the definition of “similar” contracts to include: “(A) Multiple (at least 4) geographically dispersed (one of the facilities must be at least 1,000 miles from the other 3 facilities) facilities.” If EACH contract must meet this criteria, the GSA essentially eliminates most of the competition. As a small business with only one relevant contract that meets this criteria, but other contracts that are highly relevant across the other criteria, including a contract for operation and maintenance of a930,000squarefoot hospital complex that is among the seed tasks, we would still not qualify based on this stringent requirement. Is this a requirement PER CONTRACT, or do our 3 contracts JOINTLY need to meet this criteria?

ANSWER: This criteria may be met by more than 1 contract/task order showing at least 2 sites 1,000mi apart being performed at the same time. Therefore in the context of this question, your 3 contracts may meet the criteria.

13. Question: I am writing in response to the following text:

Multiple (at least 4) geographically dispersed (one of the facilities must be at least 1,000 miles from the other 3 facilities) facilities,

(B) Each of the facilities being at least 100,000 square feet,

(C) For maintenance and repair services,

(D) Of medical treatment facilities/complexes (defined as medical centers, hospitals, clinics, medical warehouse/support facilities, veterinary clinics, trauma center, research laboratories), and (E) At least 4 of the following: (1) response to work order/service call type work; (2) the scheduling and dispatching of personnel type work; (3) scheduled and unscheduled maintenance and repair/alteration of various equipment items and systems found in a medical treatment facility type work; (4) personnel administrative responsibilities type work; and (5) urgent or emergency facility maintenance services type work.

Will the government consider revising its evaluation criteria?

ANSWER: We have taken your request under consideration and the Government will not revise the evaluation criteria. Offerors do not need to have an existing contract with existing Government contract vehicles.

14. Question: What should Attachment 2 (Attachment 2 Price Proposal IDIQ Labor Ceiling Rates) be based on?

ANSWER: The Price Proposal IDIQ Labor Ceiling Rates should be based on business judgement using Service Contract Act (SCA) rates and Davis Bacon Act (DBA) rates as a minimum.

15. Question: Is a Small Business Subcontracting Plan to be submitted with the proposal? If so, is it applicable to Small Businesses?

ANSWER: All large businesses must submit a Small Business Subcontracting Plan with Proposal.

Small Businesses are not required to submit a Small Business Subcontracting Plan.

16. Question: Is a Bid Guarantee to be included with the proposal?

ANSWER: No. A Bid Guarantee is not required.

17. Question: In accordance with Evaluation Factors – Factor 2 Management Approach, “The Offeror shall submit a proposed management approach, including quality control plan and staffing plan (including organization chart identifying key personnel, staffing positions, major subcontractors, and suppliers). To what extent do you consider a subcontractor a major subcontractor? Is a major subcontractor someone that performs over 20%, 30%, etc. of the work?

ANSWER: Major subcontractors identified should be an important, serious, or a significant part of your management approach and not necessarily a percent of work performed.

18. Question: Is Proposal to include a Management Approach for entire contract in addition to the seven sites?

ANSWER: Yes.

19. Question: Attachment_6_Past_Performance_Questionnaire, Question 5 Discussion: Question 5 of the Past Performance Questionnaire asks if the (A) Multiple (at least 10) geographically dispersed (at least of the facilities at least 1,000 miles from another one of the at least 10 facilities) facilities.

It is unlikely that the person responding to the questionnaire will know the number, complexity, or location of the other contracts/task orders being cited by the offeror.

Will the Government revise or modify Question 5 of the Past Performance Questionnaire?

ANSWER: The Offeror is to fill out Section 1, questions 16. The Past Performance POC will complete Section II, Evaluation. The Past Performance Questionnaire has been updated and is attached to this Amendment.

20. Question: IDIQ Performance Work Statement, Page 25, Paragraph 5.9, Attachment 1 Altus AFB Performance Work Statement, Page 2, Paragraph 1.4, Tinker AFB Performance Work Statement, Page 3, Paragraph 1.6, Vance AFB Performance Work Statement Page 2, Paragraph 1.5. Optional Service #1

Discussion: Annual RPIE validations are specified in the IDIQ Performance Work Statement, site specific Performance Work Statements for Altus AFB, Tinker AFB and Vance AFB. However, attachment 8 price proposal sheets indicate RPIE inventory/validation as (Optional) for all locations.

Clarification: Please confirm RPIE inventories/validations are an optional service to be directed and performed as defined in the Optional Service #1 Performance Work Statement.

ANSWER: RPIE validation is part of Preventative Maintenance tasks. The RPIE Inventory is an Optional service priced annually, and exercised at the Government’s discretion.
