PAST PERFORMANCE SURVEY
Solicitation Number: HSCG86-13-R-CWRR01: Construct Clear Water Aircraft Rinse Rack

The above contractor has identified you or your (Agency) company as a reference for past performance information for the above referenced solicitation.
Please take a few minutes to complete the attached form DD2626, evaluating the contractor’s performance under your contract(s). It is requested that the individual(s) responsible for the administrative (post-award) oversight of the project respond to this questionnaire. Please supplement your answers with comments that may be useful to the U.S. Government in assessing the contractor's past performance. Handwritten responses are sufficient. Once completed, this evaluation will be considered source selection sensitive in accordance with FAR part 3.104.
Please submit your evaluations DIRECTLY TO THE CONTRACTING OFFICER at Tammy.K.Kao@uscg.mil no later than 2pm Hawaii Standard Time on December 30, 2013.

The following definitions are applicable rating levels for the contractor’s performance.
(O) Outstanding
Performance meets contractual requirements and exceeds many to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
(A) Above Average
Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
(S) Satisfactory
Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
(M) Marginal
Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor’s proposed actions appear only marginally effective or were not fully implemented.
(U) Unsatisfactory
Performance does not meet significant contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.
