FOR OFFICIAL USE ONLY
The Program/Activity Risk Summary IS REQUIRED IF YOU HAVE ANY RISKS THAT ARE MODERATE (YELLOW) OR HIGH (RED), as determined by the Program Director or Functional Manager. The individual risks are categorized as high (red), moderate (yellow) or low (Green) in accordance with DOD guidelines. Further, trends are indicated with arrows. If necessary, the Program/ Activity Risk Summary may be more than one page. Each Program/Activity risk results from an assessment of likelihood and consequence of occurrence. Likelihood is defined below. However, Consequence of Occurrence/further quantification of these criteria shall be made by the cognizant Program Director or Functional Manager to reflect the program phase, the assessed activity, or the reported BMDS Baseline. For example, a severe consequence for a major milestone could be a schedule slip of several months, while a severe slip for a test event could be several weeks.
| Likelihood of Occurrence: | Consequence of Occurrence: | ||||
| 1 Not Likely / Likelihood ≤ 10% | 1 Minimal impact to one or more BMDS Baselines | ||||
| 2 Low / Likelihood 10%< Likelihood ≤30% | 2 Minor impact to one or more BMDS Baselines | ||||
| 3 Likely / Likelihood 30%< Likelihood ≤ 70% | 3 Moderate impact to one or more BMDS Baselines | ||||
| 4 Highly Likely / Likelihood 70% < Likelihood ≤ 90% | 4 Major impact to one or more BMDS Baselines | ||||
| 5 Near Certainty / Likelihood > 90% | 5 Severe impact to one or more BMDS Baselines |
INDIVIDUAL RISK SUMMARY
Risk ID 1 – Internal/External Interface
CONDITION: The contractor will be required to interface with external agencies like DISA and MDA internal stakeholders such as other MDA Directorates and Associate IT Support Contractors.
IF: relationships are not established and cultivated properly with the internal and external organizations
THEN: the Agency may fail in its attempts to effectively operate with the internal and external organizations listed and impact customer services and mission.
INDIVIDUAL RISK ASSESSMENT
Risk ID #1 – Internal/External Interface
L > 90%
3 –Likely
70% 2 - Low Likelihood 10% < L ≤ 30% 1- Not Likely L ≤ 10%
Minor Moderate Major
X Likelihood Rationale:
There are numerous internal and external organizations with whom the Contractor will have to interface.
Consequence Rationale:
Failure to establish agreements and relationships may cause system failures and incur rework.

This quad chart is REQUIRED IN THE MAIN BODY OF THE BRIEF FOR EACH YELLOW AND RED RISK FROM PREVIOUS CHART, SUMMARY RISK CHART. Reporting of individual risks within MDA shall consist of the following components:
Statement of Individual Risk (CONDITION-IF-THEN statement) Individual Risk Assessment (5X5 Matrix) Risk Mitigation Plan (Waterfall Chart) Risk Dependencies Pls see individual back up charts in risk section for description and instructions for each section of this chart.
Risk ID 2 – Budget Constraints
CONDITION: MDA may experience funding/budget constraints at anytime during the life of this contract and without extended notice.
IF: efficiencies and services are not identified and prioritized in their importance and user impact
THEN: the Agency may fail to minimize the potential impacts to the user community and mission.
L > 90%
70% 2 - Low Likelihood 10% < L ≤ 30% 1- Not Likely L ≤ 10%
Minor Moderate Major
Risk ID #2 – Budget Constraints
Likelihood Rationale: Contractor may not fully anticipate current and future budget constraints and plan accordingly.
Consequence Rationale:
Failure to plan and prioritize in regards to customer impact may cause unnecessary disruption in Agency operations.

Risk ID 3 – Role Based Administration (RBA) Integration
CONDITION: The contractor will be required to integrate into the existing RBA crew structure
IF: RBA integration is not established and coordinated properly within the existing crew structure
THEN: Collaboration services/capabilities may be degraded or fail and impact customer services
L > 90%
70% 2 - Low Likelihood 10% < L ≤ 30% 1- Not Likely L ≤ 10%
Minor Moderate Major
Risk ID #3 – RBA Integration
Likelihood Rationale: Contractor may not understand the MDA RBA construct.
Consequence Rationale:
Failure to integrate into the existing RBA crew structure will disrupt processes and cause delays in customer support.

Risk ID 4 – Equipment/Room Configuration
CONDITION: The contractor will be required to provide and manage equipment and room configurations for identified MICS equipment and adhere to established MDA and MDA/IC property management processes and policy.
IF: configurations are not adequately maintained and managed
THEN: the Agency would experience delays in resolving technical issues, and the Agency, Contractors or individuals may be held accountable for lost or missing property, or required SW revisions may not be accomplished.
L > 90%
70% 2 - Low Likelihood 10% < L ≤ 30% 1- Not Likely L ≤ 10%
Minor Moderate Major
Risk ID #4 – Property Management
Likelihood Rationale: Since the Contractor will not be directly responsible for the property management process, they may neglect to follow all established procedures.
Consequence Rationale:
Failure to maintain proper equipment/room configuration records, may cause loss of accountability or SW revisions are not accomplished.

Risk ID Example: Outage during Director’s All Hands
CONDITION: The Director’s All Hands is one of 9 Annual Routine Events with bridge connections linking over 90 sites around the world.
IF: One of the participants at one site disconnects the VTC and attempts to reconnect and overloads the bridge circuit
THEN: The entire system could fail and the Agency workforce would miss important, mission-related information.
