About this file
This is a Debarment, Suspension, and Other Responsibility Matters form (Form 7.13) applicable to the City Center Sport Complex Ballfields Restoration project in Florida. The form serves as a certification document requiring bidders and contractors to attest to their eligibility and responsibility status prior to contract award. The form addresses the contractor's compliance with federal debarment and suspension regulations, ensuring that neither the contractor nor its principals, owners, or affiliates appear on the General Services Administration's exclusion lists or are subject to any debarment, suspension, or other responsibility matters that would prohibit them from receiving federal or state funding.
The form requires contractors to disclose any circumstances involving criminal convictions, civil judgments, administrative proceedings, or other matters that could impact their responsibility as a contractor. Contractors must certify that they understand the requirements and acknowledge that providing false information constitutes grounds for contract termination and potential legal action. This certification is a standard compliance requirement for public works projects, particularly those involving environmental remediation and site restoration work such as the ballfields project, which includes excavation, soil replacement, and management of legacy landfill conditions. Submission of the completed form is mandatory as part of the bidding process.
