About this file
This is a Buy American Certificate form (FAR 52.225-2, October 2022) that serves as a template for contractors to certify compliance with domestic sourcing requirements for federal procurements. The certificate requires offerors to identify all end products as either domestic or foreign, with specific documentation requirements for each category. For foreign end products that do not consist wholly or predominantly of iron or steel, offerors must indicate whether they exceed 55 percent domestic content, with an exception for commercially available off-the-shelf (COTS) items. Offerors must separately list line item numbers for domestic end products containing critical components, as defined in the related "Buy American-Supplies" clause.
This certificate template is associated with a Request for Quote (RFQ) issued by the Department of Veterans Affairs Veterans Health Administration, Veterans Integrated Service Network 19, for ultrasound equipment required at the Rocky Mountain Regional VA Medical Center. The solicitation number is 36C25926Q0357, with a response deadline of April 15, 2026. The procurement is a Service-Disabled Veteran-Owned Small Business (SDVOSB) set-aside, and evaluation will proceed in accordance with Federal Acquisition Regulation Part 25 policies governing domestic preference and Buy American compliance.
