Attachment L-15 Onsite Quality Control Manager Experience Outline
HYDRANT SYSTEM REPAIR POOL
· Name of Proposed Onsite QCM: [Must be used during performance of this contract – Do not submit this individual if you submitted him/her in the role of Superintendent for any Pool)]

· Total Years of Experience as a Quality Control Manager: [DoD experience + Commercial/Private experience. DO NOT INCLUDE EXPERIENCE AS A SUPERINTENDENT, TRADESPERSON, LABORER, PM, etc). QCM experience can be for ANY type of construction – it does NOT have to be for Fuels Infrastructure and it can be experience gained when employed by any Construction Firm.]
· Years Experience performing DoD Construction as a QCM:
· Years Experience in the Commercial/Private Sector as QCM: [Commercial/Private experience not required, but the data helps (if applicable) to support DoD experience timelines.]

· [Note 2: Round the “years” to .25, .5, or .75, as applicable.]
· [Note 3: In cases where the individual has been employed to serve both as a Superintendent and a QCM, only identify the time as QCM (remove any time where the individual served only in the Superintendent role). If the individual was dual or triple hatted (SS/QCM or SS/QCM/SSHO) then the full time on that project can be counted towards time as QCM.]

· [Must be at least $1M in repairs to JP/Jet-A type products to support military aircraft Type III, IV, or hydrant fueling systems. If submitting a “repair”, (rather than new construction), the hydrant system must have been taken out of service and returned to Operator upon completion.
· Whether submitting a repair or new construction, the work must have entailed multi-disciplinary trades that are substantially mechanical or electrical.
· Must include new construction or repairs to AT LEAST THREE of the following Components of a Type III, IV, or V hydrant system:
· Hydrant pumps, hydrant valves, hydrant pits, hydrant pump houses, hydrant filtration systems, hydrant distribution pipelines, and hydrant system controls and motor control centers.
· Note: Coatings, abatement, cathodic protection, civil works, demolition, or maintenance-type efforts are NOT relevant.]
· Identify if you are submitting the entire project, or a phase of the project. [If submitting a “phase,” summarize/identify the phase of work being submitted IAW Section L&M Paragraph 4.1.2 definition of “Project”. i.e., a single BOD’d Hydrant Section/Phase Repaired or Constructed]

· Provide a statement certifying the extent the individual served in the identified role (QCM or Superintendent) for that project: [At a minimum, they must have served in that role for a minimum of four (4) months in the field (any time between construction mobilization and BOD)].
