Attachment L-12 Site Superintendent/SSHO Experience Outline
API 653 REPAIR POOL
· Name of Proposed Site Superintendent/SSHO: [Must be used during performance of this contract. Do not submit this individual if you submitted him/her in the role of Onsite QCM for any Pool]
· Current Employer: [Must be employed by the Prime Offeror (or entity of the Prime JV Offeror)]
· Completion Date of the 30-Hour OSHA Construction Safety Course: [Must be within 5 years of this RFP]
· Total Years of Experience as a Site Superintendent: [DoD experience + Commercial/Private experience. DO NOT INCLUDE EXPERIENCE AS A QCM, TRADESPERSON, LABORER, PM, etc). Superintendent experience can be for ANY type of construction – it does NOT have to be for Fuels Infrastructure and it can be experience gained when employed by any Construction Firm.]
· Years Experience performing DoD Construction as a Site Superintendent:
· Years Experience in the Commercial/Private Sector as Site Superintendent: [Commercial/Private experience is not required, but the data helps (if applicable) to support the DoD experience timelines.]

· [Note 2: Round the “years” to .25, .5, or .75, as applicable.]
· [Note 3: In cases where the individual has been employed to serve both as a Superintendent and a QCM, only identify the time as Superintendent (remove any time where the individual served only in the QCM role). If the individual was dual or triple hatted (SS/QCM or SS/QCM/SSHO) then the full time on that project can be counted towards time as Superintendent.]

· Do NOT include incidental work around an API 653 fuel tank.]
· Provide a statement certifying the extent the individual served in the identified role (QCM or Superintendent) for that project: [At a minimum, they must have served in that role for a minimum of four (4) months in the field (any time between construction mobilization and BOD)].
