Attachment L-11 On-Site Quality Control Manager Experience Outline
GENERAL POOL
· Name of Proposed Onsite QCM: [Must be used during performance of this contract – Do not submit this individual if you submitted him/her in the role of Superintendent for any Pool)]
· Current Employer: [Must be employed by the Prime Offeror (or entity of the Prime JV Offeror)]
· Total Years of Experience as a Quality Control Manager: (DoD experience + Commercial/Private experience. DO NOT INCLUDE EXPERIENCE AS A Superintendent, TRADESPERSON/LABORER, etc). QCM experience can be for ANY type of construction – it does NOT have to be for Fuels Infrastructure and it can be experience gained when employed by any Construction Firm.)
· Years Experience performing DoD Construction as a QCM:
· Years Experience in the Commercial/Private Sector as QCM: [Commercial/Private experience is not required, but the data helps (if applicable) to support the DoD experience timelines.]

· [Note 3: In cases where the individual has been employed to serve both as a Superintendent and a QCM, only identify the time as QCM (remove any time where the individual served only in the Superintendent role). · Briefly identify in abbreviated fashion the project’s entire scope, to include non-electrical or non-mechanical work such as Containment, Cathodic Protection, Coatings, Canopies, Structural Building repairs, etc
· Clearly articulate what phase of work you are submitting for this proposal, if applicable, IAW Section L&M Paragraph 4.1.2 definition of “Project”.]
· Provide a statement certifying the extent the individual served in the identified role (Superintendent or QCM) for that project: [At a minimum, they must have served in that role for a minimum of four (4) months in the field (any time between construction mobilization and BOD)].
*Instructive language shown in [Italics]. Offerors must delete or modify instructive language. Two (2) On-Site QCM personnel are required for evaluation. Ensure two (2) projects are submitted for each On-Site QCM proposed. Do not submit more than threetwo (23) pages total for each On-Site QCM.
