PAST PERFORMANCE QUESTIONNAIRE
This questionnaire is in support of the Overhead Production Support effort at Hill Air Force Base (AFB), Davis-Monthan AFB, and Randoph AFB, including supplying skilled labor for business/management, production operations, and overhead activities to support depot maintenance. Customers with direct experience with the offeror as a prime or subcontractor on recent and relevant programs for similar requirements are requested to complete this questionnaire.
Your responses are critical in evaluating the performance of this company and weigh heavily on the source selection decision of a multi-million dollar effort. Responses will be treated as source selection information.
Offeror: Complete Section 1, then e-mail the questionnaire to the customer points of contact listed on the Past Performance Questionnaire.
Offeror’s Evaluator: Complete Sections 2-5 and e-mail to: bradley.tobler.2@us.af.mil (PCO) and joseph.laing@us.af.mil (Contract Specialist).
DO NOT PROVIDE A COPY BACK TO THE OFFEROR.
