NOTES: Use this worksheet for each contract to be identified/submitted in the past performance volume – up to three, whether applicable to firms as the prime contractor or major partner or subcontractor role, offeror to choose most beneficial mix of contracts showing relevancy. See 52.212-1 Addendum paragraph 4 and subparagraphs. Re-use this template to provide information for each contract identified. Provide frank, concise comments regarding performance.
