Attachment 5 – Past Performance Information (PPI) Sheet
Past Performance Project
Page formatting shall be IAW Section L para L1.3.
1. Contractor Name: Provide Name, Address, Point of Contact, Email, and Telephone number.
2. Contract/Project Number: Number example projects consecutively (i.e. Project 1, Project 2, 3…)
3. Contract Type: (i.e. Definite-quantity, Requirements, Indefinite-quantity (IDIQ) contracts…)
4. Program Title: (Enter full name of program)
5. Cage Code:
6. Unique Entity ID:
7. Effort: (i.e., Prime, Sub, Joint Venture, Other-explain)
8. Contractor Basic Information: Provide project location, award amount, award date, period of performance, and completion date (completion date must be before OPS RFP proposal due date).
9. Description of the Project: Brief description of the effort. Provide sufficient detail (200 words or more) to establish the relevance to the technical and performance subfactor and the Minimum Team Compliment (MTC) IAW PWS section 4 requirements. If the portion of the contract was not direct with the end-user, then the example project experience is not considered to be Prime Contractor Experience.
10. Project End User: Provide project owner or end-user, such as a Government agency or installation, an institution, corporation, or private individual. Provide the name, title, and telephone number (including area code) of an individual associated with the end-user or the organization contracted for the project who is familiar with the project and the firm’s performance.
11. Example Project Attachments: Any documents, transition plans, hiring and retention plan, Minimum Team Complement (MTC): The MTC is defined as the required minimum amount of staffing necessary for effective performance on each task order, etc. required to establish relevance to the technical subfactor.
