Attachment L-2 Commodity Sample Task

STATEMENT OF WORK (SOW)

300 Chemical Biological Radiological Nuclear (CBRN) Protective Equipment Ensemble Kits

27 February 2014 Revision 1, 31 July 2014

(Updated paragraphs 1, 4 and 5.)

1.0 – General: To meet this requirement for the items identified below, United States Agency for International Development (USAID) Office of U.S. Foreign Disaster Assistance (OFDA) Logistics requires the procurement capability of the Air Force Contract Augmentation Program (AFCAP) contract to source the items referenced in Paragraph 4.0 and deliver to the OFDA Warehouse in Italy within 60 days from date of task order award. The requested items can be procured globally. The Government anticipates SIMULATED award of a firm, fixed price task order for this effort.

2.0 – Objective: Procure 300 CBRN protective equipment ensemble kits as specified in section 4.0 below.

The contractor shall deliver these items within 60 calendar days of task order award. The contractor shall provide the specific logistics data for each item, including the vendor name, address, item delivery date, point of contact information, and any other relevant details necessary to enable basic logistical planning.

The contractor shall deliver all items to the US Agency for International Development (USAID) /OFDA warehouse located at Camp Darby, Pisa, Italy. The USAID/OFDA Camp Darby POC information will be provided upon award.

3.0 – Scope: This project includes procurement for the following items:

300 CBRN Protective Equipment Ensemble Kits as listed in the paragraph 4 table. A single kit contains one suit; one mask; two filters; one pair of outer gloves; one pair of inter gloves; and one pair of protective boot covers.

5.0 – Schedule, Data Items, and Other Deliverables: The required items are as follows: 300 CBRN Protective Ensemble Kits shall be delivered no more than 60 calendar days after date of task order award.

Items shall be sent to the OFDA warehouse at Camp Darby, Pisa, Italy. The contractor shall provide the specific logistics data for each item, including the vendor name, address, item delivery date, point of contact information, weight/cubed and any other relevant details necessary to enable basic logistical planning. OFDA warehouse personnel will accept deliveries Monday through Friday from 0900 hrs to 1600 hrs local time. A single shipment is preferred, but not required. If more than one shipment, the contractor shall provide all logistic planning data for each shipment. All shipments must be “FOB Destination.”

6.0 – Preferred/Known Potential Vendors (for informational purposes only): None. OFDA will accept all items priced from vendors globally.
