ORGANIZATIONAL CONFLIT OF INTEREST
I certify that neither I nor my spouse or minor children, to the best of my knowledge, possess a disqualifying financial interest in any offeror, subcontractor, or supplier (including parent and subsidiary corporations) that is proposing on the acquisition identified above now being considered by the Source Selection Team, of which I am a member or an advisor. Financial interest includes a present or planned employment relationship. Should my aggregate financial interests (including the interests of my spouse and minor children) in the companies involved in the performance of the subject contract [CONTRACT NUMBER TBD] increase to more than $15,000 for publicly traded stocks or more than $50,000 for sector mutual funds, I will immediately disqualify myself from further participation and reveal such interests to the Contracting Officer (CO).
I do/may possess a disqualifying financial interest in an offeror, subcontractor, or supplier that is proposing on or is in a way involved in the acquisition identified above now being considered by the Source Selection Team, of which I am a member or advisor. NON-DISCLOSURE AGREEMENT (NDA)

CONTRACTOR EMPLOYEES
I, [NAME OF EMPLOYEE], as an employee of [TO BE DETERMINED], a contractor performing services under prime Contract Number [TO BE DETERMINED] (''Contract'') agree not to disclose any sensitive, proprietary or source selection information provided by the Government Customer, Directorate of Force Development (AF/A1D), or the Air Force District of Washington's (AFDW) PK Contracting Directorate under the terms of this agreement. Proprietary information/data will be handled in accordance with applicable Government regulations.
I understand that information/data I may be aware of or possess as a result of my assignment under the Contract may be considered sensitive or proprietary. My responsibility for proper use and protection from unauthorized disclosure of sensitive, proprietary and source selection information is described in Federal Acquisition Regulation (FAR) section 3.104-5(b). Pursuant to FAR 3.104-5, I agree not to appropriate such information for my own use or to release or discuss such information for my own use or to release it to or discuss it with third parties unless specifically authorized in writing to do so, as provided above.
This agreement shall continue for a term of five (5) years after the completion of the contract. Upon expiration of this agreement, I understand my continuing obligation not to disclose sensitive, proprietary, or source selection information to any person or legal entity unless that person or legal entity is authorized by the Customer's contracting officer to receive such information. I understand violations of this agreement are subject to administrative, civil and criminal sanctions. I further understand that, upon execution of this agreement, I am under an affirmative duty to report any attempt by a third party or other party not authorized to receive AF/A1D or AFDW/PK Proprietary Information to the contracting officer of the subject contract.
THIS STATEMENT CONCERNS A MATTER WITHIN THE JURISDICTION OF AN AGENCY OF
THE UNITED STATES AND THE MAKING OF A FALSE, FICTITIOUS, OR FRAUDULENT
STATEMENT MAY RENDER THE MAKER SUBJECT TO PROSECUTION UNDER TITLE 18,
