About this file
This document is a Past Performance Questionnaire template for use in evaluating contractor performance on solicitation FA524026Q0009 for Hoods, Ducts, and Fire Protection Systems Maintenance & Repair Services at Andersen Air Force Base, Guam. The questionnaire is distributed to previous customers of responding contractors to provide third-party assessment of past performance, which is identified as a key selection factor in the Government's award decision. Responses must be submitted to the Government Points of Contact (Nicole Servin, Sir Rashaliq Hankerson, and Nathaniel Entilla) no later than May 8, 2026 at 12:00 P.M. Chamorro Standard Time.
The questionnaire requests detailed contract identification information including contractor CAGE code, contract number, contract type, program title, period of performance, and dollar value, along with customer agency identification and respondent contact details. Section 4 evaluates contractor performance across six primary areas using a four-point rating scale (Excellent, Satisfactory, Unacceptable, Not Applicable): maintenance of Performance Work Statement standards, proactive response to difficulties and emergencies, provision of sufficient qualified manpower, ability to meet schedules and response times, customer service quality, and overall likelihood of rehire. The questionnaire specifically addresses three scope-of-work elements relevant to the solicitation: preventative maintenance of hoods and ducts systems, preventative maintenance and certification of fire protection systems, and repairs and service calls for these systems. This template is designated as Source Selection Information under FAR 2.101 and 3.104.
