About this file
This document is Attachment 4 of a solicitation for Hoods, Ducts, and Fire Protection Systems Maintenance & Repair Services (FA524026Q0009) and provides additional instructions to offerors submitting quotations.
The contract will be awarded as a firm-fixed price (FFP), indefinite delivery, indefinite quantity (IDIQ) contract. Quotations must be submitted via email to three designated points of contact and must remain valid for 90 days following the receipt deadline. Offerors are limited to one quotation per entity, and quotations must demonstrate clear understanding of requirements with specific, convincing evidence rather than merely restating contractual language. Quotations should be submitted in four volumes: Volume I (Complete Quotation on official letterhead, maximum 10 pages in Times New Roman 12-point font), Volume II (Pricing Information using the mandatory Attachment 2 worksheet), Volume III (maximum three Past Performance Fact Sheets with corresponding Past Performance Questionnaires submitted directly by references), and Volume IV (Representations and Certifications from SAM.gov). Each volume requires two copies—one complete and one sanitized with all company identifying information removed except the CAGE code. Volume I must include fire protection technical solutions, certifications per applicable publications, a detailed hoods and ducts maintenance plan with inspection and cleaning procedures, a quality control plan, a safety plan, and teaming agreements if subcontractors are used. All quotations must be in English with pricing in US dollars, and offerors must maintain current SAM registration through award.

The offeror shall submit a maximum of three (3) Past Performance Fact Sheets (PPFS), Attachment 7. Information on each PPFS should closely relate to the scope, size, and complexity of the proposed work under this solicitation. To demonstrate relevancy, the offeror shall compare the past performance work completed with the work description in the Performance Work Statement (Attachment 1). Provide the most current information for the Points of Contact (POCs) identified on the PPFS. At least two of the following (in descending order of availability) should be identified as current POCs:

Each Past Performance Fact Sheet should have a corresponding PPQ. The offeror shall send out and track the completion of the PPQ with the POCs identified. The responsibility to send out and track the completion of the PPQs rests solely with the offeror. A completed PPQ must be submitted directly to the Government not later than the date and time established in the solicitation for receipt of quotations. Completed PPQs submitted by the offeror will not be accepted nor evaluated.
2.4.2 Subcontractor Past Performance: If the offeror intends to use subcontractors to perform work under this contract, the offeror is encouraged to also submit Past Performance Fact Sheets and corresponding PPQs for those subcontractors. Subcontractor PPFS and PPQ submissions are subject to the same recency and relevancy criteria applicable to the principal offeror and shall be submitted in accordance with the same instructions above. Submissions for Subcontractors shall not exceed the overall maximum for PPFS listed in section 2.4. 2.5 Volume IV — Representations and Certifications
Offerors shall be actively registered in the System for Award Management (SAM) at https://sam.gov at the time a quotation is submitted and shall maintain active registration through award. By submission of a quotation, the offeror shall submit a statement asserting that all representations and certifications required by statute or regulation and maintained in SAM are current, accurate, complete, and applicable to this solicitation as of the date of quotation submission. The Government will rely on the representations and certifications maintained in SAM for purposes of eligibility and responsibility determinations. Failure to maintain current and accurate representations and certifications in SAM may render an offeror ineligible for award.
