About this file
This is a Past Performance Questionnaire for Request for Quotation No. FA524026Q0009 covering Hoods, Ducts, & Fire Protection Systems Maintenance & Repair Services at Andersen Air Force Base, Guam. The questionnaire is used by the Government to evaluate contractor past performance as a key selection factor in awarding a purchase order. References work performed by contractors within the last three years from the solicitation issuance date. The document is marked as Controlled Unclassified Information (CUI) and constitutes Source Selection Information under FAR 2.101 and 3.104.
The questionnaire requires respondents (past customers of the bidding contractor) to complete four sections: contract identification including contractor name, CAGE code, contract number, type, program title, services description, performance period, dollar value, and contractor role; customer or agency identification including name, description, and geographic location of work; respondent identification including name, title, contact information, length of service on the contract, and other points of contact; and performance information addressing scope of work and contractor performance ratings. The scope of work section asks whether contracts included preventative maintenance of hoods and ducts systems, preventative maintenance and certification of fire protection systems, and repairs and service calls. Performance ratings use a four-level scale (Excellent, Satisfactory, Unacceptable, Not Applicable) across six evaluation criteria: ability to maintain contractual standards, proactive response to difficulties, manpower sufficiency and qualifications, schedule adherence and response times, customer service, and willingness to rehire. Completed questionnaires must be returned to three Government Points of Contact by May 1, 2026 at 12:00 P.M. Chamorro Standard Time.
