MED located at Luke Air Force Base, AZ to identify lead-based paint. This report establishes lead concentrations in painted surfaces as a general guidance tool for the purpose of conducting safe renovation or de~nolitiorl activities at the building.

The information in this report or portions thereof, rnay be required to be included in no,tifications to the residents, employees, contractors, or other visitors to the building. This report is not intended to be used by the Owner or its agents as a specification or work plan for any of the work si~ggested or recommended herein.

This was a screening suwey only, so not all painted surfaces in all spaces were recorded. Represer~tative surfaces in selected spaces were sampled. Users of this report are urged to review all available results for the building before determining actual lead content of paint in any particular space. Just because a particular space is not listed in this report does not mean that there is notlor may not be lead paint in that space.

The following table lists the sample results of each painted si.rrPace sofled by location. 88 multiple sarnples of a painted surfac:e were obtained, the highest reading for I:hat surface is used ta determine its I-IUDIEPA s,katus.

The "HUDIEPA Lead Status" column indicates whether the material meets their definition of lead-based paint (LBP). Lead content i r ~ excess of 1 microgram per centimeter squared (mglcm2) by XRF testing or 5,000 parts per million (pprn) by atomic absorption analysis is consisrlered lead*-based paint by HUD and the EPA.

'The "'lead Detected" colun-~n indicates if XFiF analysis detected any amount of lead for OSHA compliance purposes. OSHA defines lead paint as "paint containing organic lead" and regulates it under OSHA Lead in Construction Standard.

'1"he XWF amit used for ,this suwc"2y irradiates the paint oru a giver1 sa~dace? causing the Bead in the paint, if present, to emit a characteristic-, freqt~ensy of )(muray radiation. The instrument identifies and counts these x-rays to determine a lead concentration. The inter~sity of this radiation is measured by the detector and is related to the amount of lead in the paint. The lead concer~tlratiican results are repsded in micrograms per square centimeter (rng/cm2).

Wl-~ile ,the sample is beirlg measured, the lead concentration is displayed 017 the XRF's display. ''$he reading and spectn~rn data are stored in tlae device. -The XRF ar~alyaer provided information on the "K-shell" (high energy) and 'I-shell" (low energy) lead. In most XRF analyzers, the K-shell lead reading reflects the amount of lead present in the older layers of pain,t, and the L--shell reading reflects the arnomrrt in the sulrFace layers of paint.

I\r8easur'ements vvere taken at points representative of all paint and varrais%\ed sudaces in the areas inspected. In order to obtain a reading, the XRF analyzer is placed with the face of the instrument flush against the surface to be tested. It is then held in place for the duration of the sample taking, approximately 20 source seconds or until the measurernent has reached an acceptable range of accuracy as determined by the inspector. The sarnpling time is dependant on the age of the radioactive source inside the XRF and the accuracy desired by the inspector. The radioactive source ,for the Nitors XL. Spectrum Analyzer used at this pm~perty is 10 ~nillicurie Cadrnium '1 89. As the Cadmium109 source ages, longer sampling times are neeessay to maintain the same level of accuracy.

Re: Medical Building 1130 1st floor asbestos containing materials:

Dear Mr. Paul:

Continental Envirotech, Inc. (CEI) is pleased to provide Environmental Programs Flight, Luke A.F.B.

with this updated asbestos survey letter for the above referenced site. There have been additional materials located at the site during demolition activities. These materials consist of HVAC rope gaskets (located between walls @ duct penetrations). The addition of this material is being added to the original inspection performed by CEI on July 30, 2007. The following materials have been identified as ACM for the project and consistent with the first floor demolition activities.

HVAC mastic and seam mastic: located in the mechanical rooms in various areas.

Monocoat / Fireproofing: previously tested positive. Additional testing by CEI and the original sampling indicated all negative results. The fireproofing was abated by the base and all subsequent sampling has been negative.

HVAC rope gaskets: These gaskets were discovered on the third floor during the first phase of construction. They have been added to the original inspection through the NESHAP update survey conducted on February 13, 2009. These materials have been addressed during demolition activities as they come in contact with them. They have been removed using the non disturbance glove bag technique.

TSI pipe hangers: TSI pipe hangers were discovered via additional sampling on the first phase of the first floor. All pipe hangers were removed via non disturbance glove bag technique.

For more information regarding the asbestos survey originally performed by CEI on July 30, 2009, please consult CEI Project Report No. 07-13164.
