| A.3 | Offerors shall submit redacted versions of their technical proposals void of any identifiable information to assist in the speed, efficiency, and fairness of the government evaluation process. | ||

| B.2.b | Offerors shall include the following sections in the Price Proposal Volume from the RFP. | ||
| B.2.b.(1) | Offerors shall complete the SF 1442 as applicable and sign and date. | ||
| B.2.b.(2) | Complete and submit any applicable clauses requiring information to be filled. | ||
| B.2.b.(3) | Offeror shall verify in the proposal that it has completed, within the last 12 months, the On-Line Representations and Certifications Application (ORCA) at http://www.sam.gov. | ||
| B.2.b.(4) | Offerors shall submit an Executive Summary as part of the price proposal of no more than 3 pages total which summarizes the key points from the offeror’s Technical, Past Performance, and Price areas. | ||
| B.2.b.(5) | Authorized offerors shall provide the following information. Failure to do so may result in the offeror’s being eliminated from further consideration. | ||
| B.2.b.(6) | Offerors shall complete the highlighted areas of Attachment 26, Financial Information Request, provide it to their financial institution, and instruct them to complete. | ||
| B.2.b.(7) | Offerors shall complete the highlighted areas of the Bonding Information Request, Attachment 27, provide it to their surety with instructions to complete, and provide it as a portion of the price proposal. | ||
| B.2.b.(8) | Provide Section 8(a) status verification from the cognizant Small Business Administration (SBA) office. | ||
| B.3 | Offerors shall ensure that the proposed Past Performance Information is submitted on a CD or DVD plus 2 hard copies. | ||
| B.3.b.(1) | Offererors shall provide a list of no more than five (5) recent contracts (current or previous) performed for Federal, State, or Local Agencies, and/or commercial customers within the last three (3) years from the date of this RFP. | ||
| B.3.b.(2) | Offerors shall submit a list of the references no later than Block 13A. | ||
| B.3.b.(3) | If a Joint Venture/teaming arrangement is contemplated, offerors shall provide complete information as to the arrangement, including any relevant and recent past performance information on previous teaming arrangements with the same partner. | ||
| B.3.b.(3) | If this is a first time joint effort, each party to the arrangement shall provide a list of past and present relevant contracts. | ||
| B.3.b.(3) | In the event the Joint Venture/teaming arrangement has less than three (3) past performance contracts, the 8(a) offeror shall submit their own past performance contracts as a supplement. | ||
| B.3.b.(4) | Offerors shall provide a letter of consent for all subcontractors for PP information. | ||
| C | Offerors shall ensure that the sealed envelope or package used to submit the proposal shows the postmark time and date of receipt, the RFP number, and the name and address of the offeror. | ||
