SECTION GC
GENERAL CONDITIONS
| PART 1 | GENERAL |
| 1.01 | DEFINITIONS: |
A. C.O. ‑ Where the term “Contracting Officer” or “C.O.” appears in the Contract Documents, it shall be understood to mean “Contracting Officer or his/her Authorized Representative”.
| B. | USER ‑ Where the term “USER” appears in the Contract Documents, it shall be understood to mean the occupant of the building. All changes, deviations, or deletions shall be made by the Contracting Officer or his/her Authorized Representative NOT the USER. |
| 1.02 | EXECUTION, CORRELATION AND INTENT: |
A. In cases of discrepancy concerning quality and application of materials and non‑technical requirements over materials, the Specifications shall take precedence over the Drawings.
B. Work shall be executed as per the Contract Documents. No changes shall be made therein without first having received written permission from the C.O.
C. If the Contractor observes any errors, discrepancies or omissions in the Contract Documents, he shall promptly notify the C.O. requesting clarification. If the Contractor proceeds with work affected by such errors, discrepancies or omissions, without having received such clarification, he does so at his own risk. Any adjustments involving such circumstances made by the Contractor, prior to approval by the C.O., shall be at the Contractor’s risk and the settlement of any complications or disputes arising there from shall be at the Contractor’s sole expense.
All work and material shall be the best of the respective kinds specified or indicated. Should any workmanship or materials be required which are not directly or indirectly called for in the Specifications and/or shown on the Drawings but which are necessary for proper fulfillment of the obvious intent thereof, said workmanship or materials shall be the same or similar parts that are detailed, indicated or specified, and the Contractor shall understand the same to be implied and provide for it in his tender as fully as if it were particularly described or delineated.
1.03 LABOR AND MATERIALS:
A. When requested by the C.O., the Contractor shall deliver to the C.O., prior to final acceptance of the work as a whole, signed certificates from suppliers of materials and manufactured items stating that such items conform to the Contract Documents.
B. The Contractor, immediately upon Notice to Proceed (or where shop drawings, samples, etc., are required, immediately upon receipt of approval thereof) shall place orders for all materials, work fabrication, and/or equipment to be employed by him in that portion of the work contracted for. The Contractor shall keep all materials, work fabrications and/or equipment specified and to advise the C.O. promptly, in writing, of all orders placed and of such materials, work fabrications and/or equipment which may not be available for the purposes of the Contract.
| C. | Mechanics whose work is unsatisfactory to the C.O. to be careless, incompetent, unskilled or otherwise objectionable shall be dismissed from work under the Contract upon written notice from C.O. |
| 1.04 | SHOP DRAWINGS, PRODUCT DATA, SAMPLES AND COLOR BOARDS: |
A. Submission of Shop Drawings, Samples and Color Boards to the C.O. is required of only those items specifically mentioned in the Specification Sections. If Contractor submits Shop Drawings for items other than the above the C.O. will not be obliged to review them. Contractor shall be responsible for the procuring of Shop Drawings for his own use as he may require for the progress of the Work.
B. The term “Shop Drawings” as used herein also includes but is not limited to fabrication, erection, layout and setting drawings, manufacturer’s standard drawings, descriptive literature, catalogs, brochures, performance and test data, wiring and control diagrams, all other drawings and descriptive data pertaining to materials, equipment, piping, duct and conduit systems and methods of construction as may be required to show that the materials equipment or systems and the positions and layout of each conform to the Contract requirements. As used herein the term “manufactured” applies to standard units usually mass-produced and “fabricated” means items specifically assembled or made out of selected materials to meet individual design requirements. Shop Drawings shall establish the actual detail of all manufactured or fabricated items; indicate proper relation to adjoining Work; amplify design details of mechanical and electrical equipment in proper relation to physical spaces in the structure; and incorporate minor changes of design or construction to suit actual condition.
C. Samples: Following Contractor’s review and approval, he shall submit to the C.O. samples of all materials in quantities and sizes determined by the C.O. If a sample is rejected, one sample noted so will be returned to the Contractor. If a sample is marked “Approved except as Noted”, one sample so noted will be returned. Corrected samples shall be resubmitted for approval as per the original submittal.
D. Color Boards: Following Contractor’s review and approval, he shall submit to the C.O. Color Boards consisting of all interior and exterior finish material samples, a finish schedule, floor plan, and interior elevations if needed to explain finishes. Consolidated Color Boards should be submitted within 30 days of Notice to Proceed. Piecemeal submittals are not acceptable. All Color Boards should be submitted in 8 ½” by 11” card stock in a three‑ring binder. Contractor must certify availability of items at this time. Color Boards shall be submitted in accordance with Section 1300 ‑ Submittals.
E. Brochures: Following Contractor’s review and approval, he shall submit to the C.O. four (4) copies of all manufacturer’s catalogs or brochures as required by the C.O. If a brochure is stamped “Approved Except As Noted”, one (1) copy will be returned. If stamped “Rejected as Noted”, one (1) marked copy will be returned. Corrected copies shall be resubmitted for approval as per the original submittal.
F. Equipment Lists: Following Contractor’s review and approval, he shall submit to the C.O. four (4) complete lists of all major items of mechanical, plumbing, and electrical equipment and materials as defined in the specification sections. Submit all items at one time in a neat and orderly manner. Partial lists will not be acceptable. Submittals shall include the manufacturer’s specifications weights space requirements, physical dimensions, ratings of all equipment and supplemental information requested by the C.O. Performance curves for all pumps and fans shall be furnished. Where a submittal sheet described items in addition to that item being submitted, the submitted item shall be clearly marked on the sheet and superfluous information shall be crossed out.
| G. | Manufacturer’s Instructions: Where any item or work is required by Specification to be furnished, installed or performed in accordance with a specified product manufacturer’s instructions, Contractor shall procure and distribute the necessary copies as specified on the Schedule of Material Submittal to the C.O. |
| 1.05 | CUTTING AND PATCHING OF WORK: |
A. In all cases exercise extreme care in cutting operations, and perform such operations under adequate supervision by competent mechanics skilled in the applicable trade. Openings shall be neatly cut and shall be kept as small as possible to avoid unnecessary damage. Careless and/or avoidable cutting damage, etc., will not be tolerated, and the Contractor will be held responsible for such avoidable or willful damage.
All replacing, patching and repairing of all materials and surfaces cut or damaged in the execution of the Work shall be performed by experienced mechanics of the several traders involved. Such replacing, repairing or patching shall be done with the applicable materials, in such a manner that all surfaces so replaced, etc., will, upon completion of the Work, match the surrounding similar surfaces.
1.06 SCAFFOLD, STAGING, PROTECTION, ETC:
A. The Work under each Section of these Specifications shall include providing, installing, and maintaining all scaffold, staging, trestles, and planking necessary for the work under each Section in strict conformity with all applicable laws and ordinances, and maintenance of same so as not to interfere with or obstruct the work of other trades.
B. All Scaffolding shall comply with OSHA Safety and Health Standards (29 CFR 1910), paragraph 1910.28.
C. Additionally, the work under each section of these Specifications shall include providing all forms of protection necessary to preserve the work of other trades free from damage.
D. These provisions shall be considered as though repeated under each separate Section of these Specifications.
1.07 SPECIFICATIONS:
A. In the preparation of these Specifications, an effort has been made to segregate the various branches of the Work under headings by trades. This is done only for convenience and shall not relieve the Contractor of the responsibility of furnishing each item indicated or specified whether properly segregated or not.
B. Specification arrangement is in accordance with the “Construction Specifications Institute (CSI), Manual of Practice, and Specification Writing Techniques, Chapter one, Section Format Outline.”
C. No responsibility will be assumed by the Government or the C.O. for omissions or duplications by the Contractor in the completion of the Contract due to any alleged error in the arrangement of the material in these Specifications nor shall any such segregation or work and materials operate to make the C.O. an arbiter in defining limits to the agreements between the Contractor and his subcontractors or suppliers.
D. The misplacement, addition or omission of any letter, word or punctuation mark shall in no way damage the true spirit, intent or meaning of these Specifications.
E. The words “shown”, “indicated”, “noted”, “scheduled” or words of that effect shall be understood to mean that reference is made to the Drawings accompanying these Specifications.
| F. | Where reference herein is made to colors or finishes “as selected”, the reference is to the C.O. |
| 1.08 | APPROVED APPLICATORS: |
| A. | Where specific instruction in these Specifications requires that a particular product and/or materials be installed and/ or applied by an “approved applicator” of the manufacturer, it shall be the Contractor’s responsibility to insure that any subcontractors used for such work be approved applicators. |
| 1.09 | WATERTIGHT ‑ WEATHERTIGHT: |
| A. | Anything in the Contract Drawings notwithstanding, the Contractor accepts the responsibility of construction of a watertight, weather tight project. |
| 1.10 | DELIVERY AND STRORAGE OF MATERIALS: |
A. Deliver all manufactured materials in the original packages, containers or bundles (with the seals unbroken) bearing the name or identification mark of the manufacturer.
B. Deliver fabrication in as large assemblies as practicable and where specified to be shop‑primed or shop‑finished; they shall be packaged or crated as required to preserve such priming or finish intact and free from abrasion.
C. Store all materials in such manner as necessary to properly protect same from damage, as materials or equipment damage by handling weather, dirt or from any other cause will not be acceptable.
| D. | Store materials so as to cause no obstructions, stored off sidewalks, roadways, and underground services. The Contractor shall be responsible for protecting all material and equipment furnished under the Contract. |
| 1.11 | WORKMANSHIP: |
A. Where not more specifically described in any of the various Sections of these Specifications, workmanship shall conform to all of the methods and operations of best standards and accepted practices of the trade or trades involved, and shall include all items of fabrication, construction, or installation regularly furnished or required for completion (including any finish), and for successful operation as intended.
B. All work shall be executed by mechanics skilled in their respective lines of work.
| C. | When completed, all parts shall have been durably and substantially built and shall present a neat, workmanlike appearance. |
| 1.12 | EARTH MOVING PERMIT: |
| A. | Contractor shall apply for and obtain an Earth Moving Permit from: |

B. Contractor shall pay for all fees for Earth Moving Permit and Application.
C. Permit is required for all Earth Moving, Soil Disturbance, and Dust Generation.
EXCAVATION:
A. Contractor shall apply for and obtain a Government Excavation Permit (AF Form 103, Base Civil Engineer Work Clearance Request) prior to any excavation.
B. There is no charge for the permit.
C. The following are the procedures for initiating and obtaining an AF Form 103 clearance by the Contractor:
The Contractor takes the government furnished AF Form 103 and the working Site Plan to the Communications Squadron building 176, Plans Flight Office. The Communications Plans Flight will perform their own clearance review of the construction area and will comment on the AF Form 103 as to when the Government communications lines will be marked. A maximum of three (3) duty days will be allowed for the marking.
The Communications Squadron notifies the Contractor to pick up the coordinated AF Form 103, and bring it to building 343, Civil Engineer Customer Service desk. At this point the person behind the desk will either direct you to the person in charge of the digging permit program or they will take your permit and put it in their IN box. Add an estimated time of 10 working days for the Civil Engineering Shops to complete this process.
The Contractor shall notify Blue Staking for Commercial Communications lines. Blue staking will issue a control number that will be annotated on the AF Form 103. The Contractor shall refer to this number for any further communications with Blue Staking.
It is the Contractor's responsibility to maintain the markings on the construction site, i.e., only approved markings, such as "rooster tails" in landscaped areas will be allowed for Contractor marking of construction.

1.14 MILITARY MISSION:
A. Luke AFB has a military mission and has restricted areas. DO NOT cross any red lines.
B. Training exercises are periodically conducted and include gate closures, vehicle searches, and identification verifications. The exercises are required for the military mission. Please cooperate during these exercises.
| C. | High-ranking officers, foreign dignitaries, or DOD employees may approach you. Only the Contracting Officer can make changes to the contract. |
| 1.15 | GOVERNMENT INSPECTION: |
A. The Government is not obligated to inspect a Contractor’s work or to protect a contractor from the consequences of such work.
B. Government inspection is a general examination of the Contractor’s conduct and work is solely for the purpose of the Government.
C. Government inspection is not to be construed as conclusive.
| D. | Information that may be offered to the Contractor does not change the Contract. Only the Contracting Officer can change the Contract. |
| 1.16 | CONTRACTOR INSPECTION: |
Contractor inspection is a careful investigation of all work to assure that it conforms to the Contract, and to detect variances and to act to correct them in time to prevent reworking of the Contract.
This includes detailed, skillful examination and testing with immediate comparison to the requirements of the Contract.
On discovery of a variance the Contractor shall immediately institute corrective action to eliminate the variance and to ensure that all future work conforms to the requirements of the Contract.
1.17 ENVIRONMENTAL REQUIREMENTS:
AIR: Excavation - When disturbing more than a tenth of an acre, a dust
Control permit will be obtained from Maricopa County Environmental Services Department. Equipment (generators, abrasive blasting, and solvent/degreaser tanks) that remains located on the base for more than 30 days will be listed and provided to the Air Program Manager.
B. ASBESTOS: No asbestos containing materials will be used on any government installation, Luke AFB, Gila Bend AFAF, or Fort Tuthill. A Material Safety Data Sheet (MSDS) is required for all building materials except for wood, glass, or steel. The MSDS will be submitted to the C.O. within seven (7) calendar days after the NOTICE TO PROCEED (N.T.P.)
C. HAZARDOUS MATERIALS MANAGEMENT: Contractor will provide the Contracting Officer (CO) the Material Safety Data Sheets (MSDS) for chemical materials to be used during the project prior to the contract award in accordance with FAR Clause 52.223-3. The CO will provide MSDSs to 56 CES/CEAN for review prior to the contractor bringing products on base. The contractor must inventory all chemical materials, including but not limited to: solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils, brought on base. A chemical material inventory must be maintained when used containing the nomenclature, manufacturer, part number, quantity, storage location, and the process in which the material is used. All chemical materials stored at Luke AFB will be managed in accordance with federal, state, local, and Air Force regulations. Long-term contracts (6 months or longer) will be managed in accordance with the installation’s approved hazardous materials (HAZMAT) tracking system. Short-term contracts (6 months or less) will report monthly HAZMAT inventories/usage to the CO.
LEAD-BASED PAINT: Paint containing lead will not be used during any painting process. A MSDS will be submitted for all paint products used.
PLUMBING FIXTURES TRIM: No plumbing fixture trim will be installed at Luke AFB unless they conform to the voluntary standard NSF/ANSI Standards 24 and 61. Products certified under these standards are tested to ensure no leaching from the fixtures exceeds 11 parts per billion of lead.
POTABLE WATER: The contractor is responsible to insure that all potable water connections have an ASSE approved anti-siphon device attached to the hose bibb prior to mixing chemicals, etc. Any anti-siphon device or backflow prevention device installed by the contractor as part of his work will be identified to the Backflow Prevention Manager, 56 CES/CEOIU, for addition to the base inventory. The type of device selected for installation and the installation location will be approved by the Backflow Prevention Program Manager prior to installation.
WASTE WATER: Absolutely no chemical products will be allowed to be dumped in any drainage system, storm drain, wash rack, septic system or sanitary sewer. All remaining and residual chemicals will be taken away by the contractor. If there are questions concerning specific waste water products, contact 56 CES/CEAN.
Solid Waste: Report monthly the amount of construction and Demolition (C&D) material generated and recycled to 56 CES/CEAN. C&D debris is defined as material produced during the construction, renovation, demolition or deconstruction of residential and commercial buildings and their infrastructure. C&D debris typically includes concrete, wood, metals, gypsum wallboard, asphalt, and roofing material. If the contractor generates a potential waste or hazardous waste during the contract period, the contractor will contact the base Hazardous Waste Manager at (623) 856-6501 for proper reporting and disposal procedures.
Environmental, Safety and Occupational Health Management System (ESOHMS): Per Executive Order 13101 and the installations Environmental, Safety and Occupational Health Management System (ESOHMS) policy, contractors must be aware of all significant aspects that fall within the installations ESOHMS. Contractors must complete ESOHMS awareness training prior to commencement of any activities. The training is provided on the Luke external webpage for review by the public and all contractors at http://www.luke.af.mil/CommunityInterests/EnvironmentalExcellence.asp
PESTICIDES: The prospective contractor and or subcontractor must operate in compliance with all state and local regulations. The contractor shall comply with all applicable parts of Title 29, Code of Federal Regulations (CFR), Occupational Safety and Health Standards, Part 1910; Title 29, CFR, Safety and Health Standards for Federal Service Contracts, Part 1925; Title 40, CFR, Parts 150-189, and Title 49, CFR, Hazardous Material Regulations, Part 171, while on an Air Force installation, to ensure safe working conditions for contract personnel and a safe environment for the occupants of Air Force Facilities.
The contractor shall furnish labels and MSDSs for all pesticide materials 25 days prior to intended use. All pesticides must be approved by AETC. Proposed pesticides must be on the Armed Forces Pest Management Board (AFPMB) standard pesticides list available at the following link. http://www.afpmb.org/pubs/sdandardlists/dod%20pesticides%20list.pdf
No pesticide shall be used without approval from 56 CES/CEOHE Entomology.
Contractor must report any pesticide use for the moth 7 calendar days prior to the end-of-month to 56 CES/CEOHE Entomology for submittal of monthly Pest Management Report.
This information is in addition to any hazardous material report submitted.
1.18 Computer Software Requirements: The contractor shall obtain licensed copies of the following standards.
The government standard for word processing, spreadsheets, presentations, and database is Microsoft Office.
The government standard for projects management is Microsoft Project or Primavera Shuretrack.
The government standard for drawings is AutoCAD.

1.19 COMPLIANCE WITH UNIFIED FACILITIES CRITERIA (UFC 3-260-01, Attachment 15 – Maximum Equipment Height and Notification of Construction Activity)
Contractor shall provide the maximum height of construction equipment to be used during construction, this data to be incorporated into the contract documents.
The maximum height of construction equipment must also be included on the Notice of Proposed Construction or Alteration (FAA Form 7460-1) to be submitted to the Federal Aviation Administration (FAA) by the project contractor.
If the maximum equipment height penetrates any airspace, it shall be noted in the contract documents and Notice of Proposed Construction or Alteration.
The FAA Notice of Proposed Construction or Alteration must be submitted at least 30 days prior to start of construction.
Notice of Proposed Construction or Alteration (FAA Form 7460-1) should be filed with the FAA using the FAA Public website at www.oeaaa.faa.gov. The site will guide you through the steps to file the automated FAA 7460-1

About the Program The purpose of the Asbestos National Emission Standard for Hazardous Air Pollutants (NESHAP) Program is to protect public health from exposure to regulated asbestos-containing material (RACM) during NESHAP facility renovation/demolition activities, asbestos removal, transport and disposal, closely monitoring those activities for proper notification and asbestos emissions control. Asbestos is known to cause cancer and other respiratory diseases in humans.
Under Section 112 of the Clean Air Act (CAA), Congress gave the U.S. Environmental Protection Agency (EPA) the responsibility for enforcing regulations relating to asbestos renovations and demolitions activities. The CAA allows the EPA to delegate this authority to state and local agencies. Even after the EPA delegate’s responsibility to a state or local agency, the EPA retains authority to oversee agency performance and to enforce the Asbestos NESHAP regulations as necessary.
The Asbestos NESHAP program in Arizona is enforced by federal, state, and county Asbestos NESHAP agencies.

Tribal Lands The federal Asbestos NESHAP coordinator who is based in the EPA Region 9 office in San Francisco has sole jurisdiction over all 25 tribal lands in Arizona.
State Level:

However, ADEQ maintains Asbestos NESHAP notification forms for renovation and demolition activities for jurisdictional counties.

"Asbestos" is a term used to describe six naturally occurring incombustible minerals. Three of the most common types are chrysotile, amosite, and crocidolite. Asbestos is a mineral compound of silicon, oxygen, hydrogen, and various metals. Asbestos minerals formed millions of years ago when heat, pressure, or chemical activity changed the physical and chemical characteristics of pre-existing rock. Unlike other minerals, which consist of tightly bound crystals, asbestos minerals are characterized by the presence of densely packed bundles of fibers.
Asbestos was a popular component in commercial products from the early 1900s to the 1970s. Asbestos is durable, fire retardant, resists corrosion, and insulates very well. Asbestos products use in America was greatest from the 1940s until the late 1970s when the health hazards associated with asbestos exposure became widely recognized. By this time asbestos had become an integral component of approximately 3,600 commercial products. During World War II, enormous quantities of asbestos were used in shipbuilding and other industries. Following the war and until the late 1970s, asbestos was widely used in buildings for fireproofing, thermal and acoustical insulation, condensation control, and decoration. It has been estimated that approximately 30 million tons of asbestos have been used in the construction and manufacturing industries since the early 1900s.
What are the Health Effects of Asbestos Exposure?
Asbestos fibers can cause serious health problems. If asbestos fibers are inhaled, these tiny microscopic fibers can cause normal functions of the lungs to be disturbed. Exposure increases the risk of developing lung cancer, mesothelioma, or asbestosis, which is a scaring of the lungs that leads to breathing problems. It could take anywhere from 15 to 30 years after the first exposure for symptoms to occur. Medical investigations have shown that inhalation and ingestion are the principal routes of entry that leads to asbestos-related diseases. There is no known safe exposure level to asbestos. The greater the exposure, the greater the risk of developing an asbestos-related disease.
Asbestos Products Ban and Phase Out On July 12, 1989, EPA issued a final rule banning some, but not all, asbestos-containing products. Regulations Governing Asbestos Pursuant to the Clean Air Act of 1970, EPA established the Asbestos National Emission Standard for Hazardous Air Pollutants (NESHAP). It is intended to minimize the release of asbestos fibers during activities involving the handling of asbestos. It specifies work practices to be followed during renovation, demolition, and other abatement activities when friable asbestos is involved. On March 31, 1971, EPA identified asbestos as a hazardous pollutant. On April 6, 1973, EPA first enforced the asbestos NESHAP Regulation in Title 40, Code of Federal Regulations, Part 61. On Nov. 20, 1990, EPA re-promulgated the entire Asbestos NESHAP regulation to enhance enforcement and compliance, Title 40, Code of Federal Regulations, Part 61, Subpart M, and Asbestos. This is the current edition. On June 17, 1994, EPA added Appendix A to clarify the Asbestos NESHAP as it affects roof removal operations involving asbestos. What sources are covered by the Asbestos NESHAP?
Among others, the following activities and facilities are regulated:
Milling of asbestos
Commercial manufacturing of products that contain commercial asbestos Demolition of all facilities, even though it may not contain asbestos Renovation of facilities that contain friable asbestos-containing materials Spraying of asbestos-containing materials Processing (fabricating) of any manufactured products that contain asbestos Use of insulating materials that contain commercial asbestos Disposal of asbestos-containing waste generated during milling, manufacturing, renovation, demolition, spraying, and fabricating operations Active waste disposal sites Closure and maintenance of inactive waste disposal sites Operation of and reporting on facilities that convert asbestos-containing waste material into non-asbestos material Design and operation of air cleaning devices Reporting of information pertaining to process control equipment, filter devices, asbestos generating process, etc.
Asbestos NESHAP Program and Notification Requirements Applicability Any facility undergoing renovation or demolition. "Facility" means any institutional, commercial, public, industrial, or residential structure, installation, or building (including any structure, installation, or building containing condominiums or individual dwelling units operated as a residential cooperative, but excluding residential buildings having four or fewer dwelling units); any ship; and any active or inactive waste disposal site.
Requirements Prior to beginning renovation or demolition activities of a facility, a certified Asbestos AHERA (Hazard Emergency Response Act) building inspector must thoroughly inspect the facility or part of the facility where the renovation or demolition operation will occur for the presence of asbestos, including friable and non-friable asbestos-containing materials.
For all demolitions (even when no asbestos is present) and renovations activities involving threshold amounts of regulated asbestos-containing material (RACM), provide the Asbestos NESHAP agency overseeing the project site with a NESHAP notification at least 10 working days prior to the demolition or renovation activity. Threshold amounts of RACM are:
260 linear feet or more on pipes
160 square feet or more on other facility components 35 cubic feet or more off facility components Fees There is no state (ADEQ) notification or permitting fees involved with this program for jurisdictional counties. The Region 9 Asbestos Program charges no fees for work on Tribal Lands. Maricopa, Pinal, and Pima counties have fees for their notification process. Some cities may have separate permit fees, and AHERA inspectors may charge a fee for their inspection.
Written Notification Requirement Under section 61.145(b) of the Asbestos NESHAP, a written notification is required for renovation and demolition operations. Only completed notification forms are accepted. For NESHAP activities for the jurisdictional 12 counties that ADEQ regulates, the notification should be hand or typewritten and postmarked or delivered to ADEQ no later than 10 working days (Monday through Friday) prior to the beginning of the asbestos activity or demolition. 1.20 Devices containing radioactive sources: Requirements
A. Civilian contractors bringing RAM or devices containing radioactive sources onto Luke AFB must have prior approval by the Installation RSO. The Servicing Contracting Office will ensure that all contractors are aware of this requirement. The Servicing Contracting Office will also place a standardized statement in bid solicitations requiring bidders to identify any planned use (including subcontractor use) of RAM or devices. The contractor must submit the following documentation to the Installation RSO for review, preferably as attachments to a cover letter on company letterhead:
B. Copy of the Nuclear Regulatory Commission (NRC) License or Agreement State License, along with any amendments, covering the radioactive material to be used. The license must be current as shown by the expiration date. Of note, the Installation RSO is specifically prohibited from approving use for more than 180 calendar days per calendar year if the contractor submits an agreement state license.
C. Copy of the users’ qualifications and radiation safety training guidance.
D. Radiation dosimetry results for the operator for the prior calendar year (if the contractor monitors his personnel with dosimetry).
E. A brief description of the proposed activities, including a copy of the part of the Air Force contract describing the work to be done at the base and the inclusive dates of the work.
F. Statement of storage and security requirements or other particular needs of the contractor.
G. Copies of the last two leak checks if required by license, permit, or State Agreement.
H. The name, local address, and telephone number for the responsible local representative and the name, address, and telephone number of the RSO named on the license.
I. Under no circumstances will an unlicensed radioactive source/device be used on Luke AFB. Any questions on contractor responsibility regarding this issue should be directed to the Installation RSO prior to the contractor starting any work.
Written Installation RSO approval must be granted prior to the contractor bringing the RAM on the installation. Any questions concerning contractor responsibilities should be directed to the Installation RSO at least 30 days prior to anticipated use.
Immediately notify the Installation RSO concerning any incident or accident, loss, theft, or damage to the device or any alleged radiation exposures. Outside normal duty hours, contact the Installation RSO through the 56 FW Command Post (6-5600) and request assistance from the on-call BE representative (623-580-2948).

C. Use standard drafting practices to represent such changes and include supplementary notes, legend, and detail as necessary to clearly portray the as‑built construction.
D. On completion of the work, deliver to the Contracting Officer both sets of marked drawings for approval and acceptance.
DISPOSAL OF HIGH INTENSITY LIGHT BALLAST AND TUBES
Disposal: The Contractor is responsible for the proper disposal of high intensity lighting ballast and tubes.
