MEMORANDUM FOR
The 4th Contracting Squadron at Seymour Johnson Air Force Base of the Air Force Air Combat Command is in the process of selecting a contractor for a requirement for Simplified Acquisition of Base Civil Engineering Requirements program (SABER IX).
One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have firsthand experience with an offeror, for the evaluation of the offeror's performance on those contracts.
Our areas of interest in the offeror are summarized in the enclosed questionnaire. Our schedule is extremely tight and we need your written response to the past performance questionnaires to be forwarded as soon as possible, but no later than the date the proposals are due. This schedule will allow us sufficient time to analyze the data prior to the start of negotiations.
To assist you in preparing your response and expediting your reply, the questionnaire may be filled out by hand and "faxed" to (919) 722-5424, attention SrA Devin Powner. Please call SrA Powner at (919) 722-1766 prior to transmission or if you have any questions. Your completed questionnaire will become part of the official Source Selection records.

CONTRACTOR PERFORMANCE QUESTIONNAIRE ON CONSTRUCTION CONTRACTS
1. The contractor named in paragraph 3 is being considered for award of the Simplified Acquisition of Base Engineering Requirements (SABER) IX, Indefinite Delivery/Indefinite Quantity (IDIQ) contract at Seymour Johnson AFB, NC, with a maximum capacity of $49,000,000.00 for a base and four option year periods. The contractor will be required to provide contract performance consisting of multi-discipline construction projects of varying complexity at Seymour Johnson AFB, Dare County Bomb Range, and Ft Fisher, NC.
2. A primary consideration in our selection process is the contractor’s performance on recent and relevant projects consisting of multi-discipline construction projects of varying complexity. Recent projects is defined as being performed during the past three (3) years from the date of issuance of this solicitation. This includes contracts that were awarded prior to this period, but still had performance during the past three (3) years from the date of issuance of this solicitation. Relevant past performance is contract performance consisting of multi-discipline (civil, architectural, structural, fire protection, mechanical, and electrical) design and construction projects of varying complexity ($250,000 and above utilizing at least 3 of the disciplines above ranging from new facility construction to site/utility work to facility renovation) within the recency timeframe shown above.

(N) Neutral Not applicable - No performance record identifiable; unable to provide a score.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor's proposed actions appear only marginally effective or were not fully implemented.
(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
