1. Self Scoring worksheet: V1-General: We have a JV agreement for this section but will also have a Teaming Agreement document. For column G "file name" would you prefer the document be combined and named just "agreement" or how do we fill in Column G if we have more than 1 document to add?
Please combine the JV and teaming agreement into a single document named “V1-
JVPA-COMPANY NAME”.
2. Pg. 6 Section L.3.4 Par.(a): Is it correct that any PPQ forms used must be filled out by the client and sent directly to the contact listed, and not included in the proposal submittal?
No. Please note that the PPQ is only used in place of a CPARS in cases where a CPARS Rating is unavailable. PPQ and CPARS are the evaluation factor for past performance and will be submitted with the proposal. In Pg. 6 Section L.3.4 Par.(a), the statement "will be accepted at CO discretion" means that during proposal evaluation the PPQ will be determined acceptable/unacceptable by the CO. There is no need to send PPQs in advance of your proposal. Please be sure to submit all of your required PPQs/CPARS within your proposal, they are required to be in your proposal to award points.
3. Self scoring worksheet: Suggest changing the Volumes to not list NAICS, very confusing as there is no NAICS code listed here:
4. Pg. 6 Section L.3.5 Par.(a): There is no request for formatting the file in Volume 5. Can you confirm if the proof of Prime file should be named: "VOL 5-Prime Contractor Work-Last 4 Contract Number-Company Name"
Please name the file ""VOL 5-PRIME-Last 4 Contract Number-Company Name". We'll add that nomenclature to Pg. 6 Section L.3.5 Par.(a).
5. Pg. 6,7 Section L.3.6 Par.(a): If there was no Warranty Recall in Volume 6, please confirm we can put N/A in the Self scoring matrix under File Name Column
Correct, if there was not a warranty recall answer "NO" in column C and "N/A" in Column G.
6. Pg. 6 Section L.3.4 Par.(a): If you don’t have a CPAR for a project do you get zero points under the past performance section?
Referencing Pg. 6 Section L.3.4 Par.(a): "In cases where a CPARS Rating is unavailable, a letter of recommendation signed by the awarding company or oversight manager, such as the project manager, will be accepted at CO discretion so long as there is adequate documentation and viable contact information for the projects point of contact." 4. VOL 4-PAST PERFORMANCE-Last 4 Contract Number-Company on SAM.gov is a template for this.
7. Pg. 6 Section L.3.4 Par.(a): Can the CPAR be an intermediate/Preliminary CPAR? Yes, as long as it is credible and an accurate indicator of work performance.
8. Pg. 6 Section L.3.1 Par.(a) For the table of contents, if these files are all separate PDF's going into separate file folder as requested. How do we create a table of contents when there will be no page numbers to refer to?
The "Table of Contents" area is provided as a way to organize information, list all documents and any important page numbers within those documents.
9. When is the last day to submit RFIs?
The last day to submit a RFI is May 8th at 1200 PDT. Questions not submitted via email will not be answered.
