About this file
This document contains a past performance worksheet template for offerors to submit with proposals for the Laughlin Air Force Base Base Operations Support re-compete solicitation. The template requests information on up to six relevant contracts the offeror has performed as prime contractor or subcontractor. For each, it asks for the contract agency, number, type, period and value, a brief description of work performed, and primary customer points of contact. Offerors must address technical areas and corrective actions relevant to the solicitation requirements. They must also describe the portion of work and percentage to be performed, whether as prime or subcontractor, and how the past experience relates to the new requirements. The template limits responses to seven pages each and does not count against the proposal's forty-page limit.
The related solicitation is for non-personal base operations support services at Laughlin Air Force Base in Del Rio, Texas. The 338th Enterprise Sourcing Squadron is issuing the competitive 8(a) small business set-aside solicitation numbered FA300222R0002 on behalf of the 47th Mission Support Group. Services include civil engineering, personnel, communications, and security support for over 4,000 people and 1,200 students annually. The period of performance and pricing terms are unspecified. The solicitation and any amendments will be available on SAM.gov. Offerors must register in SAM to participate and contact is restricted to the listed points of contact by email only.

PAST PERFORMANCE INFORMATION WORKSHEET
Submit information on recent contracts that you consider relevant in demonstrating your ability to perform the Laughlin Air Force Base (AFB) Base Operation Support (BOS) Re-compete requirement. Include rationale supporting your assertion of relevance. For a description of the characteristics or aspects the Government will consider in determining relevance, see Section M, Table M-5 of the RFP. For recency please see Attachment 23 – Section M Evaluation Factors for Award paragraphs M.3.4.2 and M.3.4.2.1. . Contract performance that meets these criteria will be evaluated in their entirety. Contract performance that does not meet these criteria will not be evaluated.
Offerors may include efforts accomplished to resolve problems encountered on the referenced contract as well as efforts to identify and manage program risk. Merely having problems does not automatically equate to less than Satisfactory rating since the problems encountered may have been on a more complex program, or an Offeror may have subsequently demonstrated the ability to overcome the problems. The Offeror should clearly describe management actions employed in overcoming problems and the effects of those actions, in terms of improvements achieved or problems rectified.
Provide the information requested in this submittal for each contract/program being described. Limit the number of efforts submitted to no more than a combined total of six (6) contracts. The length of each worksheet submission is limited to no more than seven (7) pages. This cover page with the instructions does not count against the seven (7)-page limit. Past Performance worksheets do not count against the page limitation (40 pages) for Volume II - Past Performance.
