Attachment L-2 Subcontractor/Teaming Partner Consent Letter for the Release of Past and Present Performance Information to the Prime Contractor Past performance information concerning subcontractors and teaming partners cannot be disclosed to a private party without the subcontractor or teaming partner’s consent. In an effort to assist the Government’s Performance Confidence Assessment Group (PCAG) in assessing your past performance relevancy and confidence, we request that the following consent letter be completed by the major subcontractors/teaming partners identified in your proposal. The completed consent letters should be submitted as part of your Present/Past Performance Volume.

The completed client authorization letters should be submitted as part of your Present/Past Performance Volume.

Offerors should send, with their list of references, a letter similar to the following authorizing the reference to provide past performance information to the Government.

PAST PERFORMANCE QUESTIONNAIRE COVER LETTER

FOR OFFICIAL USE ONLY

The 45th Space Wing of the Air Force Space Command is in the process of selecting a contractor for the Security Services Program. The purpose of the program is to provide a comprehensive uniformed security force to effectively execute and manage security operations on Cape Canaveral Air Force Station (CCAFS), Florida and its associated Florida Annexes.

One of the considerations in proposal evaluation is the verification of the offerors' past and present performance on contracts, which reflect the offeror's ability to perform on the proposed effort. We depend on information received from agencies such as yours, which have had first hand experience with an offeror, for the evaluation of the offeror's performance on those contracts.

Our areas of interest in the offeror are summarized in the enclosed questionnaire. As discussed in our initial phone contact with your office, our schedule is extremely tight and we need your written response no later than seven calendar days after your receipt of this letter. This schedule will allow us sufficient time to analyze the data prior to the start of formal source selection.

To assist you in preparing your response, the questionnaire may be filled out electronically or by hand and can be mailed or faxed to the following:
