Equal Employment Opportunity Program Contractor Report Form for Painting Services
This is an Equal Employment Opportunity (EEO) Program Contractor Report Form required for the San Mateo County Department of Public Works' Job Order Contracts (JOC 2617 and JOC 2618) for painting services at county-owned facilities. The form must be completed in full and submitted with bid proposals. San Mateo County is soliciting up to two qualified painting contractors with a Class C-33 license and a minimum of five years' continuous experience as a prime contractor on comparable projects valued between $100,000 and $1,000,000. A mandatory pre-proposal conference is scheduled for November 5, 2025, at 10:00 am in Redwood City, with questions due by November 12, 2025. Proposals are due December 10, 2025, at 2:00 pm via the eProcurement system, with bid opening at 2:15 pm the same day. The anticipated contract award date is January 27, 2026. Each Job Order Contract will have a one-year term with a maximum aggregate fiscal obligation of $1,000,000 per agreement.
The EEO Report Form requires contractors to provide a detailed racial and ethnic makeup of their company workforce across all job classifications, including the total number of employees in each category and the number enrolled in formal on-the-job apprenticeship training programs. The form captures data on employees identifying as American Indian or Native Alaskan, Asian, Native Hawaiian or Pacific Islander, Black American or African American, Caucasian, Filipino, Hispanic or Latino, and those selecting "Other" or choosing not to identify their race or ethnicity. Submission of this EEO questionnaire and workforce demographic report is a mandatory compliance requirement for all bidders. Additional requirements include a $25,000 bid bond, a $250,000 Performance Bond and Payment Bond (increasing to $1,000,000 each if aggregate Job Orders exceed the maximum contract value), comprehensive insurance coverage, and compliance with San Mateo County ordinances regarding equal employment opportunity, non-discrimination, and equal benefits. Contractors must also pay a 1% JOC System License Fee to Gordian and maintain Workers' Compensation, General Liability, and Motor Vehicle Liability insurance.
