About this file
The document is an ENG Form 6282 Site Safety & Health Officer (SSHO) Designation Letter from the U.S. Army Corps of Engineers for the Green River Lock and Dam #1 Interlock Wiring and Equipment Installation project. The form is used to designate a Site Safety & Health Officer who will oversee and ensure implementation of the prime contractor's Safety & Occupational Health (SOH) program. The form requires documentation including a signed SSHO Designation Letter, an OSHA 30-hour training card, and proof of 24 hours of SOH training within the last 3 years.
The form outlines three levels of SSHO qualification based on experience and training requirements, ranging from a Qualified or Competent Person to Construction or General Industry Levels 1 and 2. For each level, there are specific criteria for cumulative safety experience (3-5 years within the last 10 years) and ongoing SOH training (24 hours of documented training within the past five years, with maintenance training every three-year period). The form must be completed by the company's SOH program official and the designated SSHO, certifying the individual's qualifications and authority to stop work when necessary.
