About this file
This is a Contract Data Requirements List (DD Form 1423) for a Personnel Life Cycle Support Services contract issued by the U.S. Army (SFAE-PSE-HS). The contract number is W15QKN-26-R-A016, and this form documents a single data item requirement (Data Item 003) titled "Transition Management Process Plan."
The contractor must submit the Transition Management Process Plan in its own format within 90 days after contract award, with subsequent submissions as required. The plan must include the contractor's name, contract number, and task order number (if applicable). For entrance transitions on basic contracts, the plan shall detail a project management approach with schedules, key transition events/phases, activities including task order kick-off meeting execution and minutes, knowledge transfer and training from incumbent staff, employee hiring and staffing procedures, communications strategy, and Status of Forces Agreement (SOFA) approval processes if applicable. For exit transitions, the plan must provide a detailed project management approach with schedules and high-level representation of transition events, activities, and deliverables; knowledge transfer to the successor contractor (including technical and non-technical data, software rights, and licenses); and exit transition staffing plans ensuring successor access to key personnel and subject matter experts. For task order exit transitions specifically, the plan must address employee notification procedures and schedule, retention of key personnel, retention of adequate workforce during transition, and turnover of work-in-progress. Three copies of the final plan must be distributed to the Contracting Officer, Contracting Officer's Representative, and HRS Portal. All submissions shall be electronic.
