About this file
This is an Exhibit A template for Statement of Prior Experience, which requires offerors to document their relevant project experience and joint experience with major subcontractors for federal construction solicitations.
Part 1 of the exhibit requires prime contractors to submit no more than two prior project forms, while subcontractors may submit one form, with a maximum of three total forms per team. Each project must have been completed within five years of the solicitation issue date or be ongoing. Offerors must provide detailed information including company name, project/contract name, contract number and type, original and final contract values with explanations for any differences, project square footage, completion dates and schedule status, detailed descriptions of work performed directly by the firm, relevance assessment of the prior experience to the solicited project, identification of major subcontractors and their work, and points of contact from the government agency or prime contractor (up to two per project). Each Part 1 response is limited to four pages.
Part 2 applies only to prime contractors and requires identification of major subcontractors and teaming partners to be used on the solicited project, with descriptions of their principal and mutually exclusive scopes of work. Prime contractors must list at minimum one subcontractor for a major specialty trade (HVAC Systems is identified as an example) and may include up to one additional subcontracted trade or teaming partner. For each subcontractor or partner, offerors must document up to two previous or current projects completed together within the past five years, including project name, owner contact information, contract completion date or ongoing status, project size in square feet, final project value, the subcontractor's dollar value and percentage share, and brief scope of work description. All named subcontractors must complete a Subcontractor Consent and Reference Form (Exhibit B) to be submitted by the offeror.
