About this file
This document is an Air Force contract attachment (Attachment G.F.1) for tracking hazardous material usage data at Ellington AFB (EF). The form requires contractors to report hazardous materials used on-base, with different reporting frequencies based on contract duration. For contracts exceeding six months, contractors must submit the form monthly to the Government Representative, who forwards it to the HAZMART office. For contracts less than six months, submission is required only at the start and completion of work.
The document appears to be the first part of a larger form, containing fields for contractor information, AF-EESOHMIS Shop Code, contract number, and locations. The form's purpose is specifically tied to environmental compliance, requiring contractors to track and report hazardous material usage to meet Environmental Laws and Regulations. No pricing terms, set-asides, or funding requirements are mentioned in this portion of the document.
