About this file

This document is EPA Form 3510-13, a Notice of Termination (NOT) form issued by the United States Environmental Protection Agency for terminating stormwater discharge permits associated with construction activity under the NPDES Construction General Permit (CGP). The form applies to construction projects, including the City of Gallup, New Mexico's New Sunshine Substation Transmission Line Construction project. Contractors may submit an NOT form to terminate permit coverage when earth-disturbing activities at the site are completed and all conditions specified in Part 8.2.1 are met, when another operator assumes control of all site areas and obtains CGP coverage, or when coverage is obtained under a different NPDES permit. The form requires completion of five sections: approval to use the paper form from the EPA Regional Office, permit tracking information and termination reason, operator identification and contact details, project and site information, and certification by a responsible party. Submission requires an original document with an ink signature and must be submitted by mail to the Stormwater Notice Processing Center at the EPA headquarters in Washington, DC, or electronically through the designated EPA portal at www.epa.gov/npdes/stormwater/cgpenoi.

The form does not establish pricing terms, set-asides, or funding requirements, as it functions solely as a termination document for stormwater discharge permits. Completion of the form requires accurate and complete information regarding the operator's identification number, mailing address, and project site location, with certification under penalty of law by a responsible corporate officer, general partner, proprietor, or principal executive officer depending on the entity type. The form explicitly prohibits submission of photocopied signatures and requires that all sections be completed in uppercase letters within specified character limits. Contractors must obtain prior approval from the EPA Regional Office before using the paper form, as the standard submission method utilizes the electronic eNOI system. The OMB control number 2040-0004 applies to this form, and the estimated public reporting burden is approximately 0.5 hours per notice.
