About this file
This is a Disqualification Disclosure form required by the City of Pasadena, California in accordance with California Public Contract Code Section 10162, for the Construction Manager at Risk (CMAR) with Guaranteed Maximum Price for Construction of Transit Operations Maintenance Facility project. The form requires prospective bidders, their officers, and employees with proprietary interests to disclose, under penalty of perjury, whether they have ever been disqualified, removed, or otherwise prevented from bidding on or completing any federal, state, or local government project due to violations of law or safety regulations. Bidders must indicate their response with a yes or no checkbox, and if affirmative, must provide written explanation of the circumstances and attach supporting documentation.
The Disqualification Disclosure form is a mandatory qualification requirement for all proposers responding to the City of Pasadena's CMAR RFQ/P for the Transit Operations Maintenance Facility project. This two-phase project delivery process includes Phase 1 preconstruction and design-assist services, with an optional Phase 2 for construction services. The Selection Committee will evaluate proposals based on qualifications, team composition, technical approach, financial strength, safety record, local and small business status, price proposal, and interview scores, with the contract to be awarded on a best value basis. Proposers selected as finalists will be shortlisted, interviewed, and scored according to Pasadena Municipal Code Section 4.08.047 requirements for competitive selection using the CMAR alternative delivery method.
