Cost Proposal Scoring Guide Summary
This is a Cost Proposal and Scoring Guide for RFP #31786-00186 issued by the State of Tennessee's Department of Finance and Administration for fully-insured short-term disability (STD) and long-term disability (LTD-N) insurance services. Respondents must propose monthly premium rates per member for STD coverage (voluntary, employee-pay-all) and LTD-N coverage (mandatory, employer-pay-all) for calendar years 2027 through 2031, along with a service fee percentage for administering the employer's share of FICA taxes. The STD benefits will serve central state government and state higher education employees with a 60% benefit rate and 14-day elimination period, while LTD-N benefits will cover the same employee populations with a 63% benefit rate and 90-day elimination period. The cost proposal must be completed in full with all numeric figures provided in U.S. currency limited to three decimal places; any blank cells or non-numeric entries will be treated as zero. The proposal must be signed by an individual with authority to bind the respondent, with evidence of authority required if the signatory is not the President or Chief Executive Officer.
The cost proposal scoring evaluates three components: STD premiums (evaluated against an evaluation factor of 721,000 members), LTD-N premiums (evaluated against an evaluation factor of 4,466,000 members), and FICA service fees (evaluated against a factor of 21,000 per year). The solicitation coordinator will calculate the cost proposal score by dividing the lowest evaluation cost amount from all proposals by each respondent's evaluation cost amount and multiplying by 50 (the maximum section score). The state emphasizes that evaluation factors represent assessment purposes only and do not constitute volume guarantees, minimum purchase quantities, or any entitlement to the respondent. The total contract evaluation cost is derived from the sum of all three components' evaluation costs.

| COST PROPOSAL & SCORING GUIDE | ||
| NOTICE: THIS COST PROPOSAL MUST BE COMPLETED EXACTLY AS REQUIRED | ||
| COST PROPOSAL SCHEDULE— The Cost Proposal, detailed in this spreadsheet, shall indicate the proposed price for goods or services defined in the Scope of Services and FICA Tax Invoice sections of the RFP Attachment 6.6., Pro Forma Contract, for the entire contract period. The Cost Proposal shall remain valid for at least one hundred twenty (120) days subsequent to the date of the Cost Proposal opening and thereafter in accordance with any contract resulting from this RFP. All monetary amounts shall be numerical in U.S. currency and limited to three (3) places to the right of the decimal point. |
Provide your Proposed Monthly Premium Rate per member for each coverage option level in tabs "A.1. Prem STD " and "B.1. Prem LTD-N". Provide your Proposed Service Fee for the handling of the employer's share of FICA taxes, expressed as a percentage, on tab "C.1 FICA Service Fee" for calendar years 2027, 2028, 2029, 2030, and 2031. STD coverage will be voluntary, employee-pay-all benefits with STD benefits beginning January 1, 2027 for all central state government and state higher education employees. LTD-N coverage will be employer-pay-all benefits and shall begin January 1, 2027 for central state employees and state higher education employees.
This pricing schedule must be completed in full. Do not skip any cells. If the Respondent fails to detail all cost information for the services proposed as required, the State may determine the proposal to be non-responsive and reject it. Enter numeric figures only. Anything else, including blank cells or any deviated text, will be considered as a zero (0) and will be a cost of zero (0) to the State. The State cannot accept "N/A".
If the sum of the Evaluation Cost on a table equals zero, the Respondent will receive the maximum section score in the table. In this scenario, the State will use one cent (.01) in the formula listed in the table as the 'lowest evaluation cost amount from all proposals" to calculate all other proposals' point allocations
| NOTICE: |
| The Evaluation Factor associated with each compensable unit is for evaluation purposes only. The evaluation factors do NOT and should NOT be construed as any type of volume guarantee or minimum purchase quantity. The evaluation factors shall NOT create rights, interests, or claims of entitlement in the Respondent. |
This Cost Proposal must be signed, in the space below, by an individual empowered to bind the Respondent to the provisions of this RFP and any contract awarded pursuant to this RFP. If the individual signing this Cost Proposal is not the President or Chief Executive Officer, the Respondent must attach evidence to the Cost Proposal showing the individual’s authority to legally bind the Respondent.
