SLA Quarterly Scorecard Summary

This document is a Service Level Agreement (SLA) quarterly scorecard for Tennessee's short-term disability and long-term disability insurance contract (RFP #31786-00186) administered through the Department of Finance and Administration. The scorecard establishes ten key performance indicators (KPIs) that measure contractor performance across call center operations, member services, enrollment processing, reporting, and claims administration. Performance metrics include First Call Resolution rate (85% monthly average), welcome kit mailing timeliness (95% within 10 business days), Member Satisfaction Survey results (85% in year one, 90% thereafter), enrollment posting (100% within 2 business days), enrollment discrepancy resolution (100% within 2 business days), on-time reporting (100% adherence to specified timelines), Average Speed of Answer (30 seconds or less), and three annual claims metrics measuring payment accuracy (98%), processing accuracy (97%), and processing turnaround (98% within 21 days). The scorecard is structured for quarterly measurement throughout 2025, with quarterly evaluations worth 60 points for quarters one through three and 100 points for quarter four, totaling 280 annual points.

Contractor performance directly impacts compensation, with quarterly payment amounts of $30,000 for Q1 and $50,000 for Q2 through Q4, subject to at-risk payment reductions based on performance scores. When quarterly scores exceed 95%, no payment reduction is applied; scores between 90.1-94.9% result in $10,000 reductions; scores of 85-90% incur $20,000 reductions; scores of 80-84.9% incur $30,000 reductions; scores of 75-79.9% incur $40,000 reductions; and scores at or below 74.9% result in full $50,000 payment withholding. The scorecard reflects initial performance data for 2025, with Q1 showing 83.33% performance (10 of 60 points), Q2 and Q3 each at 16.67% (10 of 60 points), and Q4 at 0% (0 of 100 points), indicating significant performance deficiencies requiring corrective action and resulting substantial payment reductions totaling $130,000 against the annual base of $180,000.
