About this file

This is a Procurement File Memorandum from the Arizona Department of Health Services (ADHS) documenting the responsibility determination for a competitive sealed bidding solicitation (BPM006675) for pest control services at the Arizona State Hospital. The solicitation, conducted in accordance with Arizona Revised Statute 41-2533, sought qualified suppliers to provide comprehensive, all-inclusive pest control services for the hospital facility. The solicitation was posted on March 4, 2025, with proposal submissions due by March 18, 2025. The scope of work requires vendors to provide all necessary chemicals, materials, equipment, services, labor, travel expenses, insurances, workers' compensation fees, profit margins, and overhead costs as part of a single, all-inclusive contract. The memorandum, dated April 2, 2025, documents the procurement office's determination that City Wide Pest Control Inc. submitted a responsive and responsible bid that conforms to all material requirements.

The determination of responsibility confirms that City Wide Pest Control Inc. submitted all required documentation without taking exceptions to any terms and conditions, demonstrates the capability to meet the specifications and requirements set forth in the solicitation, and is legally qualified to contract with the State without debarment or suspension. The memorandum indicates that the procurement office recommends awarding the contract to City Wide Pest Control Inc. as being in the best interest of the State. No specific pricing information, contract term duration, renewal options, or incumbent vendor details are documented in this determination memorandum, though the complete solicitation documents would contain these details.
