Bidder's Questionnaire Summary
This is a Bidder's Questionnaire form developed by the City of Napa Public Works Department for evaluating the qualifications of bidders on construction projects. The City of Napa may request completion of this questionnaire from one or more apparent low bidders following bid opening as part of its evaluation process to determine bidder responsibility and qualification. Bidders must submit a fully completed, signed questionnaire with all required attachments within 48 hours of a City request. The form requires bidders to provide comprehensive information about their business structure, licensing, years in operation, experience with similar projects, safety records, and project history. Specifically, bidders must document their six most recently completed public works projects within the last three years, three largest completed projects within the last three years, and any projects similar in scope and character to the subject project. For each project identified, bidders must provide detailed information including project name and location, owner contact information, prime contractor and architect/engineer details, scope of work performed, contract pricing (initial and final including change orders), completion dates, time extensions granted, stop notices or mechanic's liens filed, liquidated damages assessed, and any claims or disputes.
The questionnaire also requires bidders to disclose their Experience Modification Rate (EMR) for the preceding three years, workplace injury and illness data from CalOSHA Form 300 or 300A, and any citations, fines, or prosecutions by local, state, or federal agencies for health and safety violations. Bidders must identify the individual responsible for their safety program and declare that all responses are true and accurate under penalty of perjury. Any errors, omissions, or misrepresentations in the questionnaire may result in bid rejection or contract termination. This form is designed to allow the City to thoroughly vet bidder qualifications, experience, safety compliance, and performance history before awarding contracts.
