About this file
This is a Performance Questionnaire attachment (W519TC-26-R-0008) required as part of an RFP submission to document and address past performance issues.
Offerors must submit a separate questionnaire for the prime contractor and each proposed subcontractor performing 20% or more of the effort or performing critical performance tasks. Prime BOA holders and their subcontractors must complete the form for the recency timeframe specified in the RFP. For each contract referenced, offerors must provide: description of effort performed, functional areas (Transportation/Supply/Maintenance), average annual dollar value calculated by dividing total obligation to date by years of performance, contractor's role (Prime or Sub), place(s) of performance, and period of performance with start and end dates.
The questionnaire contains two primary sections: (1) disclosure of any relevant contracts where the company experienced performance problems during the specified timeframe, requiring detailed information about the contract number, description of the issue, corrective actions taken, extent of success, mitigation plans, and points of contact who can verify corrective measures; and (2) disclosure of any recent and relevant contracts terminated for default or for cause in whole or in part, requiring the contract number, type of termination, reason for termination, and verification points of contact. The RFP explicitly notifies offerors that this questionnaire represents the single opportunity to adequately address negative past performance, and no additional opportunity will be provided during evaluation.
