About this file
This is a Past Performance Information Sheet template (Attachment S-5) that offerors must complete as part of their proposal for the USS LEWIS B. PULLER (ESB-3) FY25 Voyage Repair contract. The template requires contractors to provide details on up to three past performance references, including contract numbers, award amounts, performance periods, and whether CPARS evaluations were completed.
The form specifically requests detailed information about ship repair work similar to the current solicitation's requirements, including experience with ship systems (propulsion, electrical, communications), hull/structural work, preservation efforts, dry-dock operations, material control, quality control, facilities, and key personnel. Contractors must describe their contract management experience, schedule adherence, and handling of growth/new work. This template will be used to evaluate bidders' relevant past performance for the upcoming voyage repair work to be performed in Taranto, Italy from February 27 to March 13, 2025.
