About this file
This is a Foreign Country of Concern Attestation Form (PUR 1355) required by the State of Florida, Department of Management Services, for vendors responding to the Auditing and Related Services Request for Proposals. The form must be completed by an officer or representative of any entity submitting a bid, proposal, or reply to, or entering into, renewing, or extending a contract with a governmental entity that would grant the entity access to an individual's Personal Identifying Information. The attestation certifies that the entity is not owned by the government of a Foreign Country of Concern, is not organized under the laws of a Foreign Country of Concern, does not have its Principal Place of Business in a Foreign Country of Concern, and that the government of a Foreign Country of Concern does not have a Controlling Interest in the entity. The form requires the printed name, title, signature, and date of the authorizing officer or representative, and the signatory must declare under penalties of perjury that the stated facts are true.
The attestation is required pursuant to Rule 60A-1.020, Florida Administrative Code, and represents a mandatory compliance document for all vendors seeking to participate in this state contract opportunity. Failure to complete and submit this form as part of the proposal response may result in rejection of the bid. The definitions of "Foreign Country of Concern," "Controlling Interest," and other capitalized terms are established in Rule 60A-1.020, F.A.C., which vendors should reference for complete understanding of the compliance requirements.
